Role Scope:
In this role, you will be responsible for the management and delivery of direct nursing care to our participants in a variety of settings including, but not limited to, the clinic, adult day center, participant’s home, virtually, and SNF. About This Role

The Opportunity
We’re looking for an Executive Director to lead our Safety Net Service Line in California. The Safety Net service line includes our Street Medicine Program, our Enhanced Care Management (ECM) operations, our Medication Assisted Treatment (MAT) program, and our housing operations. You’ll manage and develop a team of leaders across California who will in turn oversee program managers and large frontline teams of Community Health Workers, Housing Navigators and other support staff who are delivering critically needed care. Schedule: Floats will have a set schedule of two or three 8 hour shifts/week where their primary responsibility will be patient care covering an assigned clinic for a day. We’re looking to connect with Nurse Practitioners passionate about women’s health for on call / per diem coverage across Tia’s LA Clinics (multi-site across Pasadena, Silver Lake, Studio City, West Hollywood, Culver City and Playa Vista). As a Nurse Practitioner - On call / Per Diem, you will be an integral part of the care delivery system. Your clinical expertise related to the full spectrum of providing affirming primary care for women including: gynecological services, primary care, procedures and holistic whole person care is welcome!
Nurse Practitioners are integral to the formation and iteration of our care model - with opportunities to provide direct patient care but also participate in technology development, interdisciplinary case studies, protocol development and innovation.
Schedule - On-call / Per diem Providers serve as standby coverage for same-day provider callouts or urgent clinic staffing needs. During any breaks in patient care, providers may support virtual care, inbox coverage, RN team support, etc.
Providers are asked to maintain availability for 1 or more on-call days per month, which may include weekday or weekend coverage. You tell us what day(s) each month you are available for on-call coverage. On-call shifts are 8 hours in length and will be between the hours of 8:00am–7:00pm. Providers must be able to report to clinic within two hours if activated. This role ensures consistent patient care, workflow execution, and coverage across LA clinic sites during staffing gaps, high-volume periods, training, vacation or emergent needs.
The Nurse Practitioner - On Call / Per Diem functions with high autonomy, strong clinical competency, and adaptability. They are expected to quickly integrate into each clinic’s daily flow and uphold Tia’s standardized workflows.

The ideal candidate is detail-oriented, service-minded, and experienced in supporting managers through employee relations matters in a fast-paced healthcare environment.
This position is approximately 80% remote, with periodic onsite presence required at our 4 Los Angeles-area clinics including Torrance, Long Beach, and the San Gabriel Valley clinics. Occasional travel may be required to our San Diego and Scottsdale clinic locations.

Education

The hourly range for this position is $30.00-35.00/hour. The actual compensation for this role will be determined by a variety of factors, including but not limited to the candidate’s skills, education, and experience.
Ability to stand, walk, and move throughout the clinic, if applicable, for extended periods; occasionally lift objects up to 25 lbs., bend, stoop, or reach as needed. Frequent use of hands and fingers for patient care and equipment operation. Must have normal (or corrected) vision and hearing and be able to respond quickly in a fast-paced clinical environment, if applicable.
