Role Scope: 

In this role, you will be responsible for the management and delivery of direct nursing care to our participants in a variety of settings including, but not limited to, the clinic, adult day center, participant’s home, virtually, and SNF.  You will serve as a critical member of the Interdisciplinary Team (IDT) and work collaboratively to complete assessments and drive forward participant care plans as “quarterback” for your panel.

• Delivers and documents nursing interventions as agreed upon in participant's care plans, promptly and accurately responding to physician orders, and correctly administering medications and therapeutic interventions. 

• Provides case management longitudinally and during transitions of care. Proactively coordinates complex patient discharges, transfers, and immediate post-discharge needs with hospital and long-term care facility case managers  

• Coordinates all aspects of care delivery including medication management, medical equipment and supplies, and specialist and diagnostic referrals  

• Triages in the outpatient setting, which includes independently initiating therapies within scope of practice and collaboratively working with a medical provider to escalate care as needed. 

• Educates participants, caregivers including family members, and team members on how to personalize and carry out care plans. 

• Aid with all wound care (including complex wounds), IV (hydration, therapies), and any additional procedures within RN scope of practice identified to meet evolving participant needs. 

• Delegates tasks to MA and Licensed Vocational Nurses within their respective scopes of practice. 

• Remotely takes after-hour calls that are triaged on a rotating schedule. 

• Performs related duties as assigned. 

Required Qualifications:  

• Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred. 

• Unencumbered Registered Nurse (RN) license required. 

• Minimum 2-4 years of experience clinically caring for medically complex or older adults’ population as an RN.   

• Minimum 1-2 year's experience in case management.  

• Strong clinical acumen in chronic disease management and complex geriatric care. 

• Demonstrates experience in management of clinical interventions: wound care, IVs, phlebotomy, colostomy/ileostomy care, etc. 

• Proof of medical clearance for communicable diseases, including a TB test. 

• Proof of all immunizations are up to date. 

• Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment. 

• Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law. 

Preferred Qualifications: 

• Alzheimer’s certification is preferred. 

• Case Management certification preferred  

• Bilingual: Spanish preferred. 

Compensation:  

We take into account an individual’s qualifications, skillset, and experience in determining final salary. The expected salary range for this position is $55.00 to $68.00/hour. The Traveling Medical Assistant is responsible for covering any MA roles as needed. They are responsible for assisting the medical staff (providers, nurses, and on-site Specialty Care Providers) in providing safe and efficient delivery of medical services at the WelbeHealth clinic, as directed. Essential Job Duties:

• Travel 100% of the time to various WelbeHealth clinic locations across Central California, supporting multiple sites based on business needs.
• Prepare participants for appointments, including setting up exam rooms, checking participants in and out, and gathering accurate medical histories.
• Perform pre-exam tests, such as measuring and documenting vital signs, and assist physicians during examinations.

About This Role

Axsome Therapeutics is currently searching for a field based Long-Term Care (LTC) Specialty Account Manager (SAM) to drive LTC commercial activities for an assigned geography by executing marketing strategies to ensure a successful launch and make a meaningful difference through the execution of commercial activities.

We’re looking for an Executive Director to lead our Safety Net Service Line in California. The Safety Net service line includes our Street Medicine Program, our Enhanced Care Management (ECM) operations, our Medication Assisted Treatment (MAT) program, and our housing operations. About the role:

We are constantly building our talent pipeline and welcome conversations with physicians for both current openings and future roles as Tia grows. We’re looking to connect with physicians passionate about providing comprehensive primary care to women for Tia’s Los Angeles area clinics (Both east and west LA). As a Physician (MD/DO) at Tia you’ll be integral to the excellent care we provide. Your clinical expertise related to the full spectrum of providing affirming primary care for women including: preventative care, holistic primary care and acute and chronic disease management and your desire to teach and collaborate plays an integral part of our clinical team. Board certification or board eligible in Family Medicine or Internal Medicine is preferred but we will consider other relevant specialties. 

Physicians are a critical member of our interdisciplinary care team - and this role affords you the opportunity to provide direct patient care but also engage in teaching and collaboration as well as the unique opportunity to partner with the Tia technical teams to contribute to the technology life cycle of the products and tools you will use to care for patients.

Schedule:  Floats will have a set schedule of two or three 8 hour shifts/week where their primary responsibility will be patient care covering an assigned clinic for a day. Clinic assignments will be shared the night before / morning of shift. When not in patient visits, Floats may also be assigned activities including but not limited to the following: provider inbox support, RN team support, clinical content creation and review, and clinical mentorship. You will provide coverage and support across multiple Tia clinics within the Los Angeles market. Nurse Practitioners are integral to the formation and iteration of our care model - with opportunities to provide direct patient care but also participate in technology development, interdisciplinary case studies, protocol development and innovation.

Schedule - On-call / Per diem Providers serve as standby coverage for same-day provider callouts or urgent clinic staffing needs. During any breaks in patient care, providers may support virtual care, inbox coverage, RN team support, etc. 

Providers are asked to maintain availability for 1 or more on-call days per month, which may include weekday or weekend coverage. You tell us what day(s) each month you are available for on-call coverage. On-call shifts are 8 hours in length and will be between the hours of 8:00am–7:00pm. Providers must be able to report to clinic within two hours if activated. This role ensures consistent patient care, workflow execution, and coverage across LA clinic sites during staffing gaps, high-volume periods, training, vacation or emergent needs.

Position Summary

The HR Generalist supports core HR operations across employee relations, policy administration, employee lifecycle changes, compliance, and HR process management. This role serves as a key resource for managers and employees by providing guidance on HR policies, performance management, employee changes, and workplace issues. 

This position supports both Modena Allergy & Asthma (clinical operations) and Orso Health (research operations), ensuring consistent HR processes and employee support across both organizations. This person will report to the Director of HR. 

The HR Generalist is responsible for maintaining accurate employee records, supporting leave and workers’ compensation processes, managing HR documentation and SOPs, and ensuring consistent application of people policies across the organization. The ideal candidate is detail-oriented, service-minded, and experienced in supporting managers through employee relations matters in a fast-paced healthcare environment. 

This position is approximately 80% remote, with periodic onsite presence required at our 4 Los Angeles-area clinics including Torrance, Long Beach, and the San Gabriel Valley clinics. Occasional travel may be required to our San Diego and Scottsdale clinic locations. 

Key Responsibilities 

Employee Relations & Manager Support 

• Serve as a first point of contact for HR-related inquiries through HR inbox triage. 

• Consult with managers on employee relations matters, including performance concerns and progressive discipline. 

• Support managers with job changes, organizational updates, and workforce planning considerations. 

• Follow up on performance improvement plans (PIPs) and assist managers in maintaining documentation and timelines. 

• Provide guidance to managers on company policies, procedures, and HR best practices. 

Performance Management 

• Coordinate and administer the annual performance review cycle across the organization. 

• Track completion and ensure documentation is maintained appropriately. 

• Provide support and guidance to managers throughout the performance management process. 

Employee Lifecycle & HR Administration 

• Process employee changes including job changes, salary adjustments, and changes in work hours. 

• Maintain accurate employee records and personnel files in accordance with company policies and compliance requirements. 

• Conduct periodic audits of employee files and HR documentation to ensure accuracy and completeness. 

Policy, Compliance & HR Processes 

• Maintain and update the Employee Handbook and HR policies. 

• Create, document, and maintain HR standard operating procedures (SOPs), including processes such as the 90-day probation period and salary request procedures. 

• Train managers on HR policies, procedures, and SOPs to ensure consistent application across the organization. 

• Monitor HR documentation and compliance requirements to ensure proper recordkeeping. 

Leave & Workplace Compliance 

• Manage employee leave processes, including coordination and tracking of leave requests. 

• Support workers’ compensation processes and maintain related documentation. 

• Ensure compliance with applicable employment regulations and company policies. 

HR Systems & Cross-Functional Support 

• Maintain HR records within HRIS systems and ensure accurate employee data management. 

• Partner with the Recruiting team on offer preparation and employee transition support during acquisitions. 

• Produce basic HR reports and assist in tracking key HR operational metrics as needed. 

Qualifications 

Education 

Associate’s or Bachelor’s degree in Human Resources, Business, Organizational Development, Psychology, or related field preferred. 

Experience 

1–3 years of HR experience, ideally in a healthcare, clinical, research, or multi-site environment. 
Experience supporting employee relations, HR operations, or HR compliance processes preferred.

The hourly range for this position is $30.00-35.00/hour. The actual compensation for this role will be determined by a variety of factors, including but not limited to the candidate’s skills, education, and experience.

We are seeking experienced Contract Negotiation attorneys specializing in the healthcare industry and with experience advising hospitals, large healthcare systems or large physician groups. This role involves overseeing complex contracting processes with a focus on vendor and payer-side agreements.

Responsibilities:

• Lead and manage the negotiation and execution of external client contracts, particularly with healthcare vendors, focusing on Master Service Agreements (MSAs), Statements of Work (SOWs), Business Associate Agreements (BAAs), Non-Disclosure Agreements (NDAs), and Requests for Proposals (RFPs).
• Provide strategic compliance advice to commercial operations and relevant functional areas, emphasizing market access and pricing issues related to Medicaid and Medicare.
• Review, draft, and consult on a variety of agreements, including consulting agreements, supply and distribution agreements, advisory, market access-related contracts (PBM, Payer, and GPO agreements), wholesaler contracts, and discount and rebate agreements.

• Utilize expertise in managed care to influence supply chain management, specialty pharma contracting, and federal/state pricing strategies.
• Demonstrate proficiency in navigating payer contracting or provider-side environments with payer-facing activities.
• Work collaboratively to negotiate agreements with wholesalers.

Desired Qualifications:

• Extensive experience in healthcare contracting and vendor management, with a strong emphasis on payer-side contract negotiation and management.
• Proven background in managed care, encompassing external client contract management, supply chain distribution, specialty pharma contracting, and federal/state pricing mechanisms.

Compensation:
