This role focuses on providing professional and nonprofessional nursing care services in accordance with physician ordersIn addition, this role focuses on performing the following Home Care Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient’s plan of care as directedProvides professional nursing care to patients. Requires a nursing license.   A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programsThe majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education.   A specialist level role that is a recognized subject matter expert in job area typically obtained through advanced education and work experienceTypically manages large projects or processes with limited oversight from manager, coaches, reviews and delegates work to lower level professionals, resolving difficult and often complex problems. 

 

Job Overview 

This position develops, implements, and oversees the wound care program; provides leadership; serves as a clinical resource and mentor, providing dependable consultation and guidance; provides consultation visits; promotes quality care and continuous performance improvement. The position carries out her/his responsibilities consistent with Agency policies, practices, mission and vision, and with instructions as set forth by leadershipAs a key member of the health care team, the position must interact courteously and effectively with patients and their families as well as with co-workers from all Agency departments, community resources, and with patients' physicians in order to facilitate safe and efficient patient care. 

 

Job Description 

Minimum Qualifications: 

1. Massachusetts RN Licensure. 

2. Wound, Ostomy, Continence Nursing (WOCN) certification. 

3. Three (3) years of Wound, Ostomy, Continence Nursing experience. 

4. Two (2) years of Home Health experience. 

 

Preferred Qualifications: 

1. Bachelor of Science in Nursing (BSN). 

Hours: Full Time, 40hrs. Weekend/Holiday Rotation Required.

Location: Lowell, MA and surrounding areas.

 

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive listOther duties and responsibilities may be assigned. 

 

1. Provides leadership, oversight, and dependable clinical guidance for the Wound Care Program, nurse managers, and nursing staff, consistent with the objectives of the P.I.C. 

2. Assists in establishing plans for care delivery, including case conferencing and referral to the Ethics Committee.  

3. Assures adequate delivery of care to patients without discrimination on the basis of race, gender, religious affiliation, etc., or ability to pay for services. 

4. Contributes to the P.I.C.; assists with the development of written plans, policies, procedures, and protocols related to wound care and treatment. Contributes to Process Improvement activities and continuing evaluation of programs and performance in the P.I.C. Develops immediate and long-range goals.  

5. Contributes to the development and evaluation of the Wound Care Program in accordance with the philosophy, objectives, and mission of the Agency.  

6. Plans and regularly conducts the Wound Care Committee. 

7. Maintains balance between program and service demands; priorities and staff resources. Assumes responsibility for implementing change effectively, providing support to staff.  

8. Demonstrates a caring and considerate response to staff needs by providing new employee orientation, training, ongoing supervision of professional staff, including home supervisory visits. Instructs staff nurses on all aspects of wound care documentation; reviews documentation for accuracy and completeness.  

9. Ensures that regulatory and licensing requirements for the provision of care are met. 

10. Provides oversight and management of wound care supply orders and collaborates with vendors in the selection of supplies for the formulary. 

11. Facilitates referrals to the program. Collaborates with primary nurses and other referral sources in referring patients to the wound care program. 

12. Consults with Continuum of Care/Discharge Planning departments and other community resources including private physician’s offices. 

13. Demonstrates lifelong learning by maintains an up-to-date awareness of trends and developments in wound care management and an awareness of Agency capabilities relative to patient needs. Demonstrates effective problem-solving skills; always analyzes the facts of a current situation when developing a plan of action. Maintains Professional Responsibilities. 

14. Contributes to the Agency’s Education Programs.  

15. Supervises staff in a firm, fair, and consistent manner; provides direction as necessary, leads by example.  

16. Communicates with all contacts, as necessary, in an effective, professional manner. Uses appropriate communication channels. Provides complete documentation which is concise, accurate and informative, according to agency policy, and which supports reimbursement. 

17. Completes physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies 

18. Consults with Clinical Manager on a regular basis, keeping manager informed about progresses and concerns.  

19. Reports Compliance concerns to the CEO or Chief Compliance Officer when applicable. 

20. Records any changes noted in patient's condition and reports to attending physician. 

21. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures. 

22. Works closely with nurse managers and staff in coordinating necessary services for patients and serves as a resources for the wound care program. 

23. Provides for professional development of the clinical nursing staff. 

24. Works collaboratively with all disciplines, facilitating productivity, cooperation and efficient operations. 

25. Develops criteria, tools, and methods for evaluation. 

26. Assures that the philosophy and objectives permit program and service adjustments as needs change. 

27. Evaluates performance of each individual in relation to established standards and that individual’s professional development. 

28. Encourages and assists personnel to continue self-development and improvement through formal education, meeting, and/or professional organizations. 

29. Maintains a reference library of material which is up-to-date and relevant to the day-to-day educational needs of the staff. 

 

Physical Requirements 

1. While performing the duties of this job, the employee is required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. 

2. The employee must regularly lift and/or move 25 lbs and occasionally lift and/or move up to 100 lbs. 

3. The employee in this position frequently communicates internally and externally. 

4. Must be able to exchange accurate information via phone, email and/or in person. 

5. Specific vision abilities apply including close vision, distance vision and ability to adjust focus. 

6. Noise level in the work environment is usually moderate. 

7. May be exposed to weather conditions prevalent at the time. 

 

Skills & Abilities: 

1. Demonstrates expertise and knowledge in various areas, including, but not limited to: Current methods of care and treatment; Policies, procedures, and protocols which are specific to the Agency; Agency Infection Control Plan; A thorough knowledge of all reimbursement criteria as it relates to wound care visits and supplies; and A thorough knowledge of all regulatory criteria. 

2. Excellent interpersonal skills. 

3. Ability to function well in very busy situations; effective at time management. 

4. Responsible and reliable. 

5. Good organizational skills. 

6. Knowledge of the Medicare Conditions of Participation. 

7. Ability to adapt to change.
