About the Job:

Assist provider as necessary; room patients, take vitals, triage patients, call in scripts, and other associated responsibilities.

Vascular Access Management: Insert, maintain, and remove peripheral IV lines

Medication Administration: Prepare and safely infuse complex treatments, including biologics medications

Patient Assessment: Evaluate patient, review pertinent laboratory findings, and verify physician medication orders prior to starting treatments.

Continuous Monitoring: Assess vital signs and closely track patient response during infusions to detect and immediately mitigate adverse reactions or complications.

Patient Education: Teach patients and their caregivers how to administer injectable biologics at home

Documentation: Maintain comprehensive electronic health records (EHR), precisely logging medication dosages, flow rates, and patient outcomes

What You’ll Do:

• Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities

• Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.

• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.

• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.

• Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).

Who You Are:

• Current NH Registered Nursing (RN) licensure is required.

• Nursing degree, is required.

• Certifications:  Current BLS/CPR training.

• Prior work experience in a healthcare setting is required.

• Epic experience, is a plus!

• IV experience
