Reporting to the Director, the Project Manager will lead, coordinate and support a portfolio of strategic initiatives that advance population health management across Southlake Health and the broader health system.Population Health Management at Southlake Health focuses on improving health outcomes, experience and value for defined populations through integrated care, partnerships, data-driven decision making, learning and system transformation. The role will translate strategy into coordinated action by establishing clear workplans, governance, milestones, accountabilities, risks, deliverables and measures of success across initiatives at different stages of development and implementation. The successful candidate will manage a diverse portfolio of interconnected initiatives that span implementation, evaluation, partnership development and knowledge dissemination.
The Project Manager will support priority initiatives, governance and reporting, student and learner activities, strategic partnerships, knowledge translation, communications and the development of practical tools and resources. The role will work closely with senior leaders, clinical and operational teams, physicians, analytics and digital health partners, academic collaborators, community organizations and other system partners to keep complex, multi-stakeholder work moving forward.

What you’ll be doing
- Lead and coordinate concurrent strategic initiatives across Population Health Management, including developing and maintaining project charters, integrated workplans, schedules, decision logs, risk and issue logs, budgets, status reports and implementation plans.
- Apply project management, quality improvement and change management practices to move initiatives through design, implementation, evaluation, learning and scale.
- Support teams in translating population health evidence and priorities into practical workflows, tools, resources and sustainable ways of working.
- Build and maintain effective working relationships across Southlake Health and with health system, primary care, community, academic, government and international partners.
- Plan and facilitate meetings, working groups, engagement sessions and collaborative decision-making processes.
- Establish clear project objectives, deliverables and performance indicators.
- Coordinate data and evaluation requirements and monitor progress, outcomes and dependencies.
- Prepare concise reports, dashboards, presentations and executive briefing materials.
- Capture lessons learned and support the adaptation and scaling of effective approaches.
- Coordinate project logistics for students, fellows and other learners, including project intake, onboarding, workplan and deliverable tracking, and connections with project sponsors.
- Support webinars, events, site visits, conferences, publications, resource repositories and other knowledge-sharing activities.
- Coordinate with Communications and other enabling teams to support initiative launches, stakeholder updates and consistent messaging.
- Maintain organized project documentation and an authoritative repository for governance records, tools, templates, project outputs and decisions.
- Monitor project scope, timelines, resources and budgets.
- Proactively identify risks, interdependencies and barriers, escalate issues appropriately, and recommend practical mitigation strategies.
What you’ll bring
- Strong project and portfolio management capability, with the ability to organize complex work, clarify accountabilities and manage multiple priorities in an evolving environment.
- Excellent facilitation, written communication and presentation skills, including the ability to prepare concise materials for senior leaders, boards, partners and diverse stakeholder groups.
- Demonstrated systems thinking and the ability to understand connections across clinical care, operations, data, research, partnerships and community-based services.
- Ability to build trusted relationships, influence without formal authority and work effectively across organizational and sector boundaries.
- Strong analytical, problem-solving and judgment skills, with attention to detail and a practical, action-oriented approach.
- Demonstrated ability to manage ambiguity, adapt plans as priorities evolve and maintain momentum across interdependent initiatives.
- Commitment to collaboration, inclusion, health equity, continuous learning and person- and population-centred care.
- Proficiency with Microsoft 365 project and collaboration tools, including Teams, SharePoint, Excel, PowerPoint and Word.
Qualifications
- Bachelor’s degree in health administration, business, public administration, population health, health sciences or a related discipline required
- Minimum five years of progressive experience in project or program management, preferably in healthcare, integrated care, population health, system transformation, research implementation or a related environment.
- Demonstrated knowledge of project management principles and processes. PMP certification or equivalent project management training and experience is preferred.
- Experience supporting complex, multi-stakeholder initiatives that involve executive leaders, clinicians, operational teams and external partners.
- Experience developing implementation plans, workplans, governance materials, executive reports, presentations and performance measures.
- Demonstrated knowledge of change management, quality improvement, implementation and evaluation approaches in healthcare.
- Experience with partnership development, board or committee support, academic collaborations, learner coordination or knowledge mobilization is an asset.
- Knowledge of population health management, integrated care, health equity, learning health systems or the Ontario healthcare environment is an asset.
- Proven organizational skills and ability to produce high-quality work while meeting timelines, budgets and organizational standards.
- Ability to work occasional off-hours or non-business hours during critical implementation periods, events or external engagements.
