Liquid Personnel is recruiting Occupational Therapist for its client's Reablement Team at Southampton, supporting adults to maximise independence and reduce ongoing care needs.

What will your responsibilities be?

• Complete OT assessments for individuals awaiting Reablement.
• Identify opportunities to maximise independence and reduce care needs.
• Assess for equipment, adaptations and moving & handling requirements.
• Recommend rehabilitation and reablement interventions.
• Contribute to care planning, risk assessments and reviews.
• Work closely with Reablement staff, therapy teams and partner agencies.
• Maintain accurate records and provide person-centred support.

Qualifications & Experience:

To be successful in this role you must have, 

• Occupational Therapy degree or equivalent.
• Current HCPC registration.
• Experience within adult health or social care services.
• Reablement or community OT experience is desirable.
• Experience undertaking robust assessments and managing risk.
• Knowledge of safeguarding and relevant legislation.
• Strong communication and multi-agency working skills.
• Full driving licence and access to a vehicle essential.
• Enhanced DBS required.
