Pleasant Hill Post Acute is seeking a compassionate, motivated, and dedicated Occupational Therapist (OT) to join our rehabilitation team.

What You'll Do

As an Occupational Therapist, you will help patients regain independence, improve functional abilities, and achieve meaningful rehabilitation goals.

Responsibilities include:

• Evaluate patients and develop individualized treatment plans based on their functional needs and goals.
• Provide skilled occupational therapy services to improve independence with activities of daily living.
• Help patients maximize functional abilities and improve their overall quality of life.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, physical therapists, speech therapists, and other members of the interdisciplinary team.
• Communicate effectively with patients, families, and caregivers regarding treatment goals and progress.
• Complete accurate, timely, and compliant clinical documentation.
• Provide compassionate, patient-centered care while maintaining professional clinical standards.
• Participate in interdisciplinary care planning and discharge planning as appropriate.

Qualifications

• Current California Occupational Therapist (OT) license in good standing.
• Strong clinical reasoning, communication, and organizational skills.
• Ability to work effectively as part of a multidisciplinary team.
• Previous skilled nursing, post-acute, or rehabilitation experience preferred.
• New graduates are welcome to apply.
• Positive attitude and genuine commitment to patient-centered care.
