Job Description Summary
Join a patient-centered, value-based care organization where the focus is on quality outcomes, preventive care, and meaningful patient relationships rather than visit volume. This full-time opportunity offers a Monday through Friday schedule (8:00 AM to 5:00 PM) primarily serving patients throughout San Gabriel Valley, Pomona Valley, and western Inland Empire, providing Nurse Practitioners with the flexibility of a mobile care model supported by care coordination, population health resources, and a collaborative interdisciplinary team. Care is delivered through home visits, community-based care, and virtual appointments, allowing you to make a meaningful impact while practicing with greater autonomy in one of Southern California's most diverse communities.
