Job Description Summary
Join a patient-centered, value-based care organization where the focus is on quality outcomes, preventive care, and meaningful patient relationships rather than visit volume. This full-time opportunity offers a Monday through Friday schedule (8:00 AM to 5:00 PM) serving patients throughout the Antelope Valley, San Fernando Valley, Conejo Valley, and Simi Valley. You'll provide acute, post-acute, and transitional care services in patients' homes, helping individuals successfully recover after hospitalization, manage chronic conditions, and avoid unnecessary readmissions. Supported by a collaborative interdisciplinary team, you'll have the autonomy, resources, and time needed to deliver personalized, relationship-centered care where it matters most.

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting.
