Family Nurse Practitioner (FNP)

Position Overview
Impact Life is seeking a Family Nurse Practitioner (FNP) to join our growing rural health team in Sussex County. Candidates who are dual-certified/licensed as both a Family Nurse Practitioner (FNP) and Psychiatric-Mental Health Nurse Practitioner (PMHNP) are strongly preferred.
The FNP will provide integrated medical services as part of Impact Life’s Rural Recovery Ecosystem Initiative, including primary and preventive care, substance use disorder treatment, medications for opioid use disorder (MOUD), medication management, and care coordination.
Working as part of a multidisciplinary team, the FNP will help connect primary healthcare, behavioral health, substance use disorder treatment, recovery support, and community-based services to address the whole-person needs of individuals served.
Essential Duties & Responsibilities
- Conduct comprehensive medical assessments, health histories, physical examinations, and ongoing clinical evaluations.
- Provide primary and preventive healthcare services within the FNP’s scope of practice.
- Assess, diagnose, and treat acute and chronic medical conditions within scope of practice.
- Assess and treat substance use disorders and co-occurring medical conditions.
- Initiate, prescribe, and manage medications for opioid use disorder (MOUD) when clinically appropriate and within scope of practice.
- Provide medication management, monitoring, and patient education.
- Order, review, and interpret appropriate laboratory tests, diagnostic studies, and other clinical information.
- Develop and participate in individualized treatment, care, and recovery planning.
- Provide health education, preventive care, and chronic disease management support.
- Identify medical and social barriers that may affect an individual’s health, treatment, or recovery and coordinate appropriate interventions and referrals.
- Coordinate care with nursing, behavioral health, substance use treatment, peer recovery, and community-based staff.
- Collaborate with external healthcare providers, pharmacies, treatment providers, and community partners to support continuity of care.
- Support transitions between healthcare, behavioral health, treatment, recovery, and community-based settings.
- Promote trauma-informed, harm reduction, person-centered, and recovery-oriented approaches to care.
- Maintain timely, accurate, and complete clinical documentation in accordance with organizational, regulatory, and professional standards.
- Maintain compliance with applicable clinical, licensing, prescribing, privacy, confidentiality, and documentation requirements.
- Participate in interdisciplinary case consultation, team meetings, quality improvement activities, and other program initiatives.
- Maintain required professional licenses, certifications, registrations, and continuing education requirements.
- Perform other duties consistent with the scope and responsibilities of the position.
Requirements
Required Qualifications
- Graduate degree from an accredited Nurse Practitioner program with preparation as a Family Nurse Practitioner (FNP).
- Current national certification as a Family Nurse Practitioner (FNP) through a recognized certifying body.
- Current Delaware APRN licensure with an FNP population focus, or eligibility to obtain Delaware licensure.
- Current prescriptive authority and DEA registration, or eligibility to obtain both.
- Ability to prescribe and manage medications for opioid use disorder (MOUD) consistent with applicable federal and state requirements.
- Strong clinical assessment, diagnostic, clinical judgment, communication, care coordination, and documentation skills.
- Ability to work effectively as part of a multidisciplinary and integrated care team.
- Experience working with individuals with substance use disorders and/or co-occurring medical and behavioral health needs.
- Knowledge of trauma-informed, harm reduction, person-centered, and recovery-oriented approaches to care.
Preferred Qualifications
- Dual certification/licensure as both a Family Nurse Practitioner (FNP) and Psychiatric-Mental Health Nurse Practitioner (PMHNP).
- Experience providing substance use disorder treatment, including MOUD.
- Experience in primary care, addiction medicine, behavioral health, community health, rural health, or integrated healthcare.
- Experience working with individuals who have complex medical, behavioral health, substance use, and social needs.
- Experience working within a multidisciplinary or integrated healthcare team.
- Experience serving rural, underserved, or historically marginalized populations.
Work Environment
This position is part of Impact Life’s Rural Recovery Ecosystem Initiative and will primarily serve individuals and communities in Sussex County, Delaware. The role requires collaboration across healthcare, behavioral health, treatment, recovery, and community-based services and may involve providing services across multiple program or community-based settings.
