Job Summary

We are seeking a compassionate and dependable Certified Nursing Assistant (CNA) to provide high-quality personal care and support to patients receiving home health and hospice services. The CNA works under the supervision of a Registered Nurse and assists patients with activities of daily living while promoting comfort, dignity, safety, and quality of life in the home setting.

Responsibilities

• Assist patients with personal care, including bathing, grooming, dressing, and toileting.
• Provide assistance with mobility, transfers, and ambulation.
• Monitor and report changes in patient condition to the supervising nurse.
• Take and document vital signs as assigned.
• Assist with light housekeeping related to patient care.
• Support patients and families with compassion and respect.
• Maintain accurate and timely documentation in EMR charting program.
• Follow infection control, safety, and agency policies and procedures.
• Travel to patient homes within the assigned service area.

Qualifications

• Current CNA certification in the State of Arizona.
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Current CPR certification or ability to obtain upon hire.
• Ability to work independently and manage time effectively.
• Excellent communication and interpersonal skills.
