Job Summary
As a Physical Therapist in Home Health, you'll enjoy the freedom and flexibility to balance your personal and professional life while helping patients regain independence and improve their quality of life.
Responsibilities
- Develop individualized treatment plans tailored to each patient's needs
- Provide hands-on therapy in the comfort of patients' homes
- Collaborate with physicians, nurses, and interdisciplinary team members
- Educate patients and families on treatment goals and progress
- Make a lasting impact on patients' health, mobility, and well-being
Qualifications
- Current and active Physical Therapy License
- Current CPR Certification
- Valid driver's license, auto insurance, and reliable transportation
- Excellent communication and interpersonal skills
- Commitment to patient-centered care
- Ability to work independently and collaboratively within a team environment
