About the Role

As a Hospice RN Case Manager, you will be at the heart of patient-centered end-of-life care. You will provide personalized support focused on comfort, dignity, and peace, partnering closely with patients, families, and our interdisciplinary care team to build and implement comprehensive, individualized care plans. Your clinical expertise and deep empathy will guide symptom management, ease transitions, and offer vital emotional support when it matters most

.Key Responsibilities

• Clinical Assessment & Monitoring: Assess patient healthcare needs, monitor symptoms, evaluate medication responses, and track overall progress.
• Care Coordination: Collaborate seamlessly with physicians, family members, and interdisciplinary healthcare professionals.
• Patient & Family Support: Provide compassionate education and emotional support to patients and their families throughout their journey.
• Team Collaboration: Facilitate and actively participate in interdisciplinary team meetings to optimize care plans.
• Documentation & Compliance: Ensure accurate, timely clinical documentation in alignment with company policies and regulatory standards.

Qualifications

• Active Registered Nurse (RN) license.
• A genuine passion for hospice and palliative care.
• Exceptional communication, empathy, and organizational skills.

Requirements

• Current and unrestricted Registered Nurse license in the state of Arizona
• Minimum of [1] years of experience in hospice or palliative care or 1-2 years of critical care
• Knowledge of hospice regulations, practices, and standards of care preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work effectively in a team environment
• Proficiency in electronic medical record systems
• Availability to work evenings, weekends, and rotating shifts as needed
