Field Operations Manager

• Schedule: Full-time, Monday–Friday, with occasional evenings or weekends as needed

• Location: Field-based within an assigned market

• Compensation: $75,000 - $90,000 (yearly) + performance-based incentive eligibility

Adaptive Home Health is building a higher-acuity, patient-centered skilled home health model across Texas. Our ultimate mission is to dramatically improve patient access to home health care.

The Field Operations Manager is responsible for ensuring patients receive timely, reliable care by overseeing daily scheduling, clinician capacity, workforce performance, and service-failure prevention. This role connects our patients, field clinicians, Patient Care Coordinators, clinical leaders, and growth teams so operational issues are resolved before they affect care.

We build technology to help our field teams operate faster and with better support. Our charting product is designed to reduce documentation burden, and our scheduling product will improve visit coordination, reduce last-minute changes, and provide better visibility into capacity and coverage.

What You Will Do

• Oversee daily scheduling, visit assignments, rescheduling, and service-failure prevention across the assigned market.

• Monitor clinician capacity and coordinate coverage for PTO, callouts, urgent visits, and changes in patient demand.

• Review clinician productivity, documentation timeliness, and operational performance; provide coaching and accountability when expectations are not met.

• Lead and support Patient Care Coordinators by delegating appropriate scheduling and reporting responsibilities while maintaining ownership of outcomes.

• Proactively resolve missed visits, outstanding starts of care, patient access issues, and other barriers that could delay care or billing.

• Support clinician onboarding and reinforce expectations related to scheduling, communication, documentation, and operational processes.

• Partner with Account Executives and clinical leaders to respond to service concerns, assess coverage feasibility, and support referral-source commitments.

• Escalate recurring or high-risk operational issues and recommend solutions that improve patient access, clinician utilization, and service reliability.

Requirements

Must-haves

• Experience managing healthcare operations, field operations, home health scheduling, or a similarly complex service-delivery environment

• Experience leading, coaching, and holding team members accountable for performance

• Strong judgment and the ability to make time-sensitive operational decisions

• Ability to manage competing priorities, urgent patient needs, and frequent scheduling changes

• Experience monitoring productivity, capacity, scheduling, or operational performance metrics

• Strong problem-solving skills with a focus on identifying root causes and implementing corrective action

• Clear, professional communication with clinicians, patients, operational leaders, and growth partners

• Ability to delegate effectively while maintaining ownership of results

• Strong organization, follow-through, and attention to detail

• Comfort addressing performance concerns and conducting direct accountability conversations

• Proficiency with scheduling systems, reporting tools, and electronic documentation platforms

• Current driver’s license, reliable transportation, and willingness to travel throughout the assigned market
