Assertive Outreach Support Worker
Perth, WA | Fixed Term Full-Time to 30 June 2027

As an Assertive Outreach Support Worker, you will build trusted relationships with some of Perth's most vulnerable community members, providing practical support and connecting people with the services they need to move towards safety, stability and long-term housing. About the role
As an Assertive Outreach Support Worker, you will work within a mobile outreach service supporting people experiencing primary homelessness throughout the Perth CBD and surrounding areas.
You will provide outreach, assessment, referrals, advocacy and practical assistance while working within a strengths-based, trauma-informed and person-centred framework. You will support individuals to access housing, health, income support and community services, while helping build trust and create positive pathways towards long term stability.
You will also contribute to coordinated community responses that support people experiencing homelessness to access sustainable housing and longer-term support.
This is a fixed term full-time position within the Assertive Outreach Team, reporting to the Team Leader. Pay is in accordance with the SCHADS Award Level 4. The position is offered on a maximum term contract until 30 June 2027 (with a possibility of extension - funding permitted).
How you will make an impact
- Build authentic and respectful relationships with people experiencing homelessness through proactive outreach.
- Assess immediate and ongoing support needs and develop appropriate responses.
- Provide practical assistance including information, referrals, transport and material aid.
- Support individuals to access housing, health, mental health, alcohol and other drug, and community support services.
- Advocate for clients and work collaboratively with government, community and housing providers.
- Maintain accurate client records, case notes and service data.
- Undertake risk assessments and contribute to safe outreach practices.
- Participate in multi-agency initiatives that support coordinated responses to homelessness.
- Contribute to a positive, inclusive and high-performing team culture.
What you will bring
- A Diploma in Community Services or a related discipline, practical experience in welfare service delivery, or significant experience working within homelessness services.
- Experience supporting vulnerable individuals experiencing disadvantage, homelessness or complex needs.
- Knowledge of trauma-informed, strengths-based and person-centred practice.
- Understanding of the challenges faced by people experiencing homelessness, mental health, social isolation and other barriers to stability.
- Strong communication and relationship-building skills with the ability to engage people from diverse backgrounds.
- Sound judgement, resilience and the ability to work effectively in dynamic environments.
- Strong administration and record-keeping capabilities.
- A current WA driver's licence.
- Current First Aid certificate
