Why this role matters 

This role offers the opportunity to walk alongside people who are facing some of the most challenging moments of their lives. Many participants come to House 49 carrying the weight of homelessness, trauma, mental health concerns, AOD use and social disconnection. As a Senior (Complex) Case Manager, you help create a space where people feel safe, respected and supported to rebuild stability and hope.

About the role 

You will provide intensive case management to participants with mediumtohigh complexity, supporting them to stabilise, engage and thrive within the Housing Support Program. You will develop individual case plans, facilitate access to health and community services, and help participants build confidence through life skills and groupbased activities. 

Working within a multidisciplinary team, you will respond to critical incidents, support referral prioritisation and contribute to a culturally safe, traumainformed environment. Your professional insight and compassionate approach help ensure participants receive consistent, highquality support that aligns with TSA’s model of care. 

This maximum term position is contracted until 31 December 2027 and sits within the Homelessness team, reporting to the Housing Support Program Team Leader. Pay is in accordance with a SCHADS Level 5. 

How you will make an impact 

  • Provide intensive case management to participants with complex and intersecting needs 

  • Support referral allocation and prioritisation based on risk, urgency and program capacity 

  • Respond to critical incidents and escalating behaviours using traumainformed approaches 

  • Complete intake and assessment to build shared understanding of participant strengths and needs 

  • Develop individual case plans that support stability, wellbeing and personal goals 

  • Assist participants to attend appointments, access services and build community supports 

  • Advocate for participants with government and nongovernment agencies 

  • Maintain accurate, uptodate records in line with TSA standards 

  • Identify risks and escalate concerns appropriately to ensure safety for participants, staff and TSA 

  • Contribute to group activities and life skills programs that build confidence and connection 

  • Work collaboratively with internal TSA services and external providers to ensure coordinated care 

What you will bring 

  • Degree qualification in Social Work, Mental Health, Welfare or Community Development, or relevant experience with a minimum Certificate IV in Community Services 

  • Demonstrated experience supporting people with multiple and intersecting needs including mental health, AOD, trauma, disability, family violence and justice involvement 

  • Experience completing risk assessments, safety plans and responding to highrisk situations 

  • Strong knowledge of Darwin’s homelessness sector and local community supports 

  • Experience working with culturally diverse communities, including Aboriginal and Torres Strait Islander peoples 

  • Ability to communicate clearly, build rapport and create shared understanding 

  • Strong organisational skills and experience managing a case load 

  • Resilience, adaptability and commitment to culturally safe, inclusive practice 

  • First Aid Certificate 

  • NT Working with children check or willingness to apply 

  • NT Driving Licence  

What we offer 

  • $53.43 – $55.85 hourly + superannuation based on experience level and qualifications, exclusive of penalties, allowances, NFP salary packaging
