About this Qualified Social Worker role at Liquid Personnel
Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s team, based in Conwy.
What will your responsibilities be?

Hospital discharge planning — Lead safe and timely discharges by completing strengths‑based assessments, coordinating MDT input, and arranging appropriate community or residential support in line with the Social Services and Well‑being (Wales) Act 2014.

Adult safeguarding — Identify, investigate, and manage safeguarding concerns for older adults and vulnerable adults, undertaking Section 126 enquiries, risk assessments, and protection planning with partner agencies.

Complex case management — Hold a varied caseload including hospital‑based referrals, cognitive decline, physical frailty, and long‑term conditions; complete Care and Support Plans, Mental Capacity Act assessments, and best‑interest decisions.

Qualifications and Experience:
To be successful in this role you must have;
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
