About this Occupational Therapist – Discharge to Assess role at Liquid Personnel
Liquid Personnel is recruiting an experienced Occupational Therapist for its client’s Thackley Green Specialist Care Centre. This is a full-time agency role offering £39 per hour and supporting rehabilitation, discharge planning, and safe transitions from hospital back into the community.
The Role
We are seeking a skilled Occupational Therapist to join a multidisciplinary team supporting patients admitted to the service following discharge from acute hospital settings. You will play a key role in delivering therapy assessments, rehabilitation programmes, home access visits, and discharge planning to help patients regain independence and return safely to their communities.
Working collaboratively with healthcare professionals, patients, and families, you will provide person-centred interventions, recommend equipment and minor adaptations, and contribute to seamless transitions of care.
What Will Your Responsibilities Be?
- Lead Occupational Therapy assessments and rehabilitation programmes.
- Assess patients admitted from acute hospital settings.
- Conduct home access visits to identify equipment and minor adaptation requirements.
- Manage a varied caseload and delegate tasks to Therapy Assistants where appropriate.
- Support the wider Discharge to Assess pathway when required.
- Work collaboratively with multidisciplinary professionals to facilitate safe and timely discharges.
- Produce accurate assessments, reports, care plans, and clinical documentation.
- Promote independence, wellbeing, and person-centred care throughout the rehabilitation and discharge process.

Qualifications and Experiences
- Qualified Occupational Therapist.
- Current HCPC registration.
- Experience in rehabilitation, discharge planning, or community Occupational Therapy.
- Strong assessment, clinical reasoning, and problem-solving skills.
- Experience managing a caseload and overseeing or delegating work to Therapy Assistants.
- Excellent communication and multidisciplinary working skills.
- Full UK driving licence and access to a vehicle with appropriate business insurance.
