We are seeking an organized, compassionate, and motivated Clinical Coordinator (LVN) to join our growing Hospice team in San Francisco. What You'll Do

As a Clinical Coordinator, you'll play a critical role in ensuring our patients receive seamless, compassionate care while supporting our clinical team and daily operations.

Patient Care Coordination

  • Serve as a key liaison between patients, families, physicians, and hospice clinicians.
  • Coordinate care services and assist with resolving patient care concerns.
  • Support communication across the interdisciplinary team.

Clinical Documentation & Orders

  • Request, track, clarify, and process physician orders.
  • Upload and manage documentation within the EMR system.
  • Ensure records are complete, accurate, and compliant.
  • Assist with clinical documentation and workflow management.

Clinical Operations

  • Support field clinicians with scheduling and care coordination.
  • Monitor and maintain medical supply inventory.
  • Ensure the supply room remains organized and OSHA compliant.
  • Assist with audits, quality improvement initiatives, and regulatory readiness.

Team Support

  • Assist with onboarding and orienting new team members.
  • Collaborate with nurses, social workers, chaplains, physicians, and leadership.
  • Contribute to a positive and supportive team culture.

Qualifications

Required

  • Current California LVN License in good standing
  • Strong communication and customer service skills
  • Excellent organizational and multitasking abilities
  • Ability to work independently and prioritize responsibilities
  • Knowledge of medical terminology
  • Proficiency with computers and electronic medical records

Preferred

  • Experience in Hospice, Home Health, Palliative Care, Skilled Nursing, or Case Management
  • BLS Certification
  • Associate Degree or higher
  • Experience coordinating patient care in a healthcare setting
