Nationally recognized stroke program with all participating hospitals earning AHA Gold Plus or Silver Plus designations. Expanding Telestroke network across the region.

Professional position requiring advanced clinical, educational and managerial skills in the provision of interdisciplinary care for the patient with cerebral vascular disease.  This position requires the knowledge and skill to design and implement clinical pathways relevant to the stroke patient in addition to evaluating patient’s progress or variance.  

Recognition of variance in the pathway as system, patient, or personnel related is a key concept in assuring optimal patients’ outcome.  The ability to analyze and recommend system and/or clinical practice changes is an integral component of this position.  The incumbent is responsible for the education of the staff caring for stroke patients in collaboration with WJMC and LCMC leadership.

DUTIES AND RESPONSIBILITIES:   

40%    
I.    Facilitating Concurrent Participation of the Interdisciplinary Team over the Continuum of Care 

A.    Leads stroke activations hospital-wide including performing rapid triage assessment and facilitating treatment/care.

B.     Participates in clinical stroke rounds with the Interdisciplinary Team
1.     Interacts as needed with members of the Interdisciplinary Team including but not limited to: physicians, nurses, therapists, pharmacists, social workers, case managers, dietitians and administrative personnel to ensure compliance with regulatory requirements.

2.    Works with the Interdisciplinary Team to keep the care team and family informed about the patient’s progress and discharge recommendations.

3.    Provides patient data, information on stroke team effectiveness and community outreach progress to the Critical Care and Stroke Committees, actively participating in committee assignments and recommendations.

C.    Educate the public through community outreach efforts using the resources available through WJMC to decrease the incidence of stroke morbidity and mortality in our patient population.

D.    Assesses, plans, implements and evaluates ongoing staff education programs to increase the general body of knowledge and to provide optimum standard of care.

E.    Work with Stroke Program Manager to optimize stroke data collection and utilization of results.

II.    Collaborates with professional organization to provide information memberships and professional certification

1.    Acts as a resource for current stroke guidelines and best clinical practice guidelines related to stroke care.

2.    Provides education and training for staff involved in the care of stroke patients.

3.    Provides staff follow-up related to compliance with best-practice guidelines.

III.    Demonstrates the ability to develop, implement, and evaluate the effectiveness of patient information education

A.     Assures that current and appropriate patient educational materials are available for use in the clinical area

1.    Assesses current information available at WJMC

2.    Develops new materials as requested by stroke program leadership

3.    Recommends procurement of materials available in the market as appropriate

B.    Provides education on stroke awareness, prevention and care at discharge

1.    Assesses the patient and/or family/significant other for readiness and ability to learn

2.     Provides teaching according to level of understanding and stage of illness 

3.    Communicates effectively with the team to assure awareness of material and resource availability for patient education

IV.     Demonstrates the ability to function as a leader in the management of the stroke patient

A.    Consults with nursing management, seeking advice as needed and providing information related to areas for improvement, corrective action, and follow-ups

B.    Serves as a resource person in improvement opportunities, exhibits advanced clinical knowledge in protocol and procedure content, application of theory to practice, and available hospital and community resources

C.    Participates in the development and maintenance of current standards of care, protocols and procedures for stroke patients

D.    Participates in management, unit staff meetings, and special committee meetings as assigned

E.    Collaborates with health team members in facilitating working relationships that result in a smooth flow of patients through the system

V.    Utilizes effective interpersonal skills

A.    Demonstrates effective communication skills for effective supervisors, co-workers, patients, significant others and other health care workers

B.    Maintains confidentiality to protect and respect the patient’s rights, privacy and property at all times

C.     Responds to others in a helpful manner

D.    Maintains a professional relationship with medical staff, health care team members, visitors and patients of all developmental and socio-cultural backgrounds    

E.     Provides information to patients and significant others which conveys an attitude of acceptance, sensitivity and caring


F.    Relays information   appropriately over the telephone, pagers and other communication devices

G.     Provides and accepts direction and constructive criticism for behavior, actions and performance

VI.     Maintains and advances professional educational level to enhance expertise

A.     Becomes active member in a least on of the following:   The American Heart Association, American Stroke Association, National Stroke Association or American Association of Neuroscience Nursing

B.     Attends conferences and educational opportunities to enhance professional knowledge in the field of neuroscience nursing and stroke management

C.     Reviews current research journals to update clinical practice for self and others
