Liquid Personnel is recruiting an Adult/ Children’s Social Worker to join its client’s Single Point of Access team, based in Flintshire. 

What will your responsibilities be? 

• Conduct safeguarding screenings, community care assessments, and risk assessments for adults, determining appropriate interventions and managing complex cases within the Single Point of Access service.
• Manage an allocated caseload, developing, implementing, and reviewing outcome-focused care and support plans that promote independence, minimise risk, and improve wellbeing.
• Work collaboratively with individuals, families, carers, and multi-agency partners to coordinate services, provide professional advice, maintain accurate records, and progress safeguarding and referral actions.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK driving licence 
• Post-qualified experience 
• Degree level or equivalent in Social Work
