Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Learning Disabilities team, based in Doncaster. 

What will your responsibilities be? 

• Provide person-centred social work support to adults with learning disabilities, promoting independence, choice, rights and wellbeing.
• Complete holistic assessments of care and support needs in line with relevant legislation and local authority procedures.
• Develop, review and coordinate personalised care and support plans with individuals, families and partner agencies.
• Safeguard adults at risk of abuse or neglect, responding appropriately to safeguarding concerns and following statutory procedures.
• Work collaboratively with individuals, families, carers and multidisciplinary professionals to achieve positive outcomes.
• Support people to access appropriate health, social care, housing, education, employment and community services.
• Advocate for individuals and ensure their views, wishes and preferences are central to decision-making.
• Undertake Mental Capacity Act assessments and contribute to best-interest decision-making where appropriate.
• Apply relevant legislation, policies and professional standards to social work practice.
• Support individuals through transitions and significant changes in their lives, including changes in accommodation, services or support arrangements.
• Manage a varied caseload, maintaining accurate and timely records and completing statutory documentation.
• Work proactively with partner organisations to develop creative and appropriate support solutions.
• Promote inclusion, equality, dignity and the rights of people with learning disabilities.
• Participate in reviews, case discussions, team meetings and other multi-agency forums.
• Contribute to continuous improvement and the development of services for people with learning disabilities.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK driving licence 
• Post-qualified experience 
• Degree level or equivalent in Social Work
