Our client based in Herefordshire is seeking an experienced Social Worker to join their Community Wellbeing and Social Work Team (CAAsT). This is an excellent opportunity for a motivated practitioner to work within a strengths-based, person-centred service, supporting adults with a wide range of needs, including older people, individuals with physical disabilities, learning disabilities, and mental health needs.

The successful candidate will manage a varied community-based caseload, undertaking Care Act Assessments, Mental Capacity Assessments, safeguarding enquiries, Continuing Healthcare (CHC) work, Section 117 responsibilities, reviews, and support planning. You will play a key role in promoting independence, wellbeing, and positive outcomes whilst working collaboratively with individuals, carers, families, health professionals, and partner agencies.

Key Responsibilities

Core duties include:

• Managing a caseload of adults with varying levels of need, undertaking assessments and interventions in accordance with statutory duties and best practice.
• Completing strengths-based Care Act Assessments to identify eligible needs, personal outcomes, risks, and appropriate support arrangements.
• Developing and reviewing person-centred care and support plans that promote independence, wellbeing, choice, and community inclusion.
• Undertaking Mental Capacity Assessments and applying the principles of the Mental Capacity Act 2005 to support informed decision-making.
• Completing safeguarding enquiries, including Section 42 enquiries, ensuring adults are protected from abuse, neglect, self-neglect, and exploitation.
• Assessing and managing risk, implementing appropriate safeguarding measures whilst promoting least restrictive practice.
• Working with adults with physical disabilities, learning disabilities, mental health needs, and older adults, providing tailored interventions to meet individual needs.
• Undertaking Continuing Healthcare (CHC) assessments and contributing to Decision Support Tools (DSTs) where required.
• Completing Section 117 aftercare work, supporting individuals transitioning from secondary mental health services and ensuring appropriate support arrangements are in place.
• Working collaboratively with individuals, carers, families, advocates, health professionals, and partner agencies to deliver coordinated and effective support.
• Supporting individuals through periods of crisis and increased vulnerability, utilising professional judgement and available resources to achieve positive outcomes.
• Coordinating services and resources with health providers, community organisations, and commissioned services to meet assessed needs.
• Undertaking regular reviews to ensure care and support arrangements remain effective and responsive to changing circumstances.
• Preparing clear and comprehensive assessments, reports, support plans, and case records in line with legislative and organisational requirements.
• Participating in multi-disciplinary meetings and professional forums to support integrated working and positive outcomes for service users.

Qualifications & Experience

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
• Registration: Must be registered with Social Work England.
• Experience: Significant post-qualified experience within Adult Social Care, preferably within a community-based setting.
• Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Continuing Healthcare Framework, Section 117 responsibilities, and associated statutory guidance.
• Assessment Skills: Proven ability to complete Care Act Assessments, Mental Capacity Assessments, safeguarding enquiries, reviews, and person-centred support planning.
• CHC Experience: Experience completing CHC assessments and participating in DST processes.
• Safeguarding Experience: Experience undertaking Section 42 enquiries and managing safeguarding concerns involving vulnerable adults.
• Multi-Agency Working: Ability to work effectively with health colleagues, providers, carers, families, advocates, and partner agencies.
• Communication Skills: Excellent communication, report-writing, analytical, organisational, and decision-making skills.
• Strengths-Based Practice: Demonstrable commitment to strengths-based social work and promoting independence and wellbeing.
• Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
