Liquid Personnel is recruiting an Adult's Social Worker to join its client’s ASC Transformation team, based in Nottingham. This is an exciting opportunity for a skilled practitioner to undertake complex reviews and reassessments across a wide range of Adult Social Care services, whilst contributing to service improvement and transformation initiatives.
Working across Older People, Physical Disabilities, Learning Disabilities, Mental Health, and Supported Living services, you will review support arrangements, assess changing needs and outcomes, and promote strengths-based, person-centred practice. Alongside frontline social work, you will play an important role in shaping new ways of working, improving processes, and supporting innovation within Adult Social Care.
What will your responsibilities be?
- Undertaking complex reviews and reassessments across a range of Adult Social Care service areas.
- Completing Care Act 2014 assessments, reviews, and support planning to ensure services remain appropriate, effective, and outcome-focused.
- Reviewing existing care and support arrangements to identify changing needs, strengths, risks, and opportunities for greater independence.
- Working with adults with physical disabilities, learning disabilities, mental health needs, older people, and individuals living in supported living settings.
- Promoting strengths-based and person-centred approaches that maximise independence, wellbeing, and choice.
- Undertaking safeguarding enquiries and responding appropriately to safeguarding concerns in accordance with statutory guidance and local procedures.
- Completing Mental Capacity Assessments and contributing to Best Interest decision-making in line with the Mental Capacity Act 2005.
- Working collaboratively with citizens, carers, families, providers, health professionals, and partner organisations to achieve positive outcomes.
- Supporting proportionate decision-making and ensuring services remain focused on individual outcomes and value.
- Preparing high-quality assessments, reviews, reports, and support plans in accordance with legislative and organisational requirements.
- Contributing to Adult Social Care transformation projects, service redesign activities, and continuous improvement initiatives.
- Supporting the implementation of innovative approaches, digital solutions, and new technologies that enhance social work practice and service delivery.
- Utilising tools such as Microsoft Copilot, Beam Notes, and other digital systems to support effective recording, reflective practice, and case management.
- Maintaining accurate, timely, and legally compliant case records and documentation.
Qualifications and Experience:
To be successful in this role you must have;
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake complex reviews, reassessments, risk assessments, and support planning.
- Safeguarding Experience: Experience managing safeguarding concerns and undertaking safeguarding enquiries.
- Transformation Experience: Experience contributing to service improvement, transformation programmes, or change initiatives is desirable.
- Multi-Agency Working: Strong experience working collaboratively with health professionals, providers, families, carers, and partner agencies.
- Digital Skills: Confident using electronic case management systems and digital tools to support high-quality social work practice.
- Communication Skills: Excellent communication, analytical, report-writing, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice, innovation, and continuous professional development.
