Position Summary

The Clinical Nurse Coordinator serves as a key member of the Rheumatology care team, coordinating and managing patient care throughout the continuum of care while acting as a clinical resource and patient advocate. The Rheumatology Division offers a variety of specialized outpatient clinics across the St. Louis region, providing individualized treatment and support for patients with arthritis, musculoskeletal disorders, autoimmune diseases, and other inflammatory conditions Experience caring for patients with autoimmune, inflammatory, musculoskeletal, or immunosuppressive conditions is strongly preferred.

Schedule: Monday-Friday 8-5pm
Locations: Provide support to 2-3 locations in the surrounding St. Louis area (North County, West County, South County, and the Center for Advanced Medicine)

Working collaboratively with physicians, advanced practice providers, patients, families, and multidisciplinary team members, the Clinical Nurse Coordinator ensures optimal patient outcomes. Responsibilities include coordinating complex care plans, facilitating communication among care teams, supporting patient education, and helping patients navigate treatment throughout their healthcare journey.

The division fosters a collaborative, patient-centered environment that promotes clinical excellence, professional growth, and teamwork. This position is well suited for a registered nurse who is passionate about building long-term patient relationships, coordinating complex healthcare needs, and contributing as an essential member of a multidisciplinary team dedicated to improving the lives of patients with chronic rheumatologic conditions.

Job Description

• Provides direct patient care, including assessment, planning, implementation and evaluation of patients; completes health history and physical examination on patients; conveys findings to physician.
• Coordinates patient care, orders diagnostic tests, conveys findings to physician, attends inpatient rounds, coordinates patient appointments and assists with referrals to outside sources.
• Assists physician and provides support with examinations, procedures and other processes related to direct patient care.
• Performs a variety of nursing administrative duties, including compliance with environmental safety regulations, infection control standards and quality assurance.
• Acts as liaison and team leader by facilitating communication and coordination of care between patients, families and health team members; explains routines and procedures to patients and coordinates patient care information to appropriate sources.

Salary Range

$64,500.00 - $109,900.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
