Our Community Care Record platform, Care Link, enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs (HRSN). Path Assist is our tech-enabled Community Health Worker program for addressing HRSN utilizing an evidence-based, structured intervention. Our goal is simple: address individuals’ unmet HRSNs, increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend.
Role Overview:
The Registered Nurse Manager leads and develops a team of RN Supervisors, each of whom oversees their own distributed team of RN Home Health Assessors, who travel to members' homes across Massachusetts to complete in-person health assessments for a managed care health plan's dually eligible members. This is a leadership role focused on building supervisory capability, standardizing clinical quality practices across teams, and ensuring the overall program consistently meets the health plan's completeness, timeliness, and quality standards. The RN Manager is the escalation point for supervisory-level issues and the senior clinical and operational authority for the assessment program, while the scope of the program itself remains assessment and documentation with a clean handoff to the health plan's care management team, not care plan development or ongoing care management.
Responsibilities:
- Leadership & Development
- Hire, onboard, train, coach, and retain RN Supervisors; set clear performance expectations and growth plans for each.
- Provide regular one-on-ones, performance management, and professional development for Supervisors, including periodic joint field observations and ride-alongs to calibrate coaching quality.
- Build bench strength and succession readiness across the Supervisor team; ensure each Supervisor maintains an effective coverage plan for their own team during PTO, leave, and turnover.
- Set and reinforce program-wide field-safety standards for independent in-home visits, ensuring Supervisors are equipping and supporting nurses working alone in the community.
- Clinical Quality & Audit Oversight
- Partner with the Quality Assurance & Improvement Specialist to implement the program's overall audit strategy, ensuring Supervisors are conducting rigorous pre-submission audits of assessments (Uniform Core, Functional, and Comprehensive), and SOAP notes across all teams.
- Monitor documentation turnaround in GuidingCare and completeness/quality performance against the health plan's standards at the program level, intervening where individual teams fall short.
- Aggregate and analyze error trends across Supervisors and their teams; direct corrective action plans, retraining, and calibration sessions to close systemic gaps.
- Oversee timely, high-quality responses to review and audit findings within contractually required timelines, escalating and resolving root causes that span multiple teams.
- Serve as the program's subject-matter expert on the assessment process, tools, and applicable regulatory, privacy (HIPAA), NCQA, and care management standards; keep Supervisors current as requirements evolve.
- Operations & Productivity
- Own program-level caseload distribution, visit scheduling, and territory/route planning strategy, ensuring Supervisors balance workload and completion targets effectively across their teams.
- Track, analyze, and report on program-wide KPIs (visits completed, assessment timeliness, accuracy, submission deadlines) by team and in aggregate; hold Supervisors accountable to targets and drive continuous improvement.
- Partner with the scheduling team on program-level capacity planning, ensuring assessor schedules stay full and efficient across all Supervisor teams.
- Serve as a primary point of contact with the health plan's program leadership through operations touchpoints and office hours, representing program performance and improvement plans.
- Own program-level compliance oversight, including workforce exclusion checks and timely completion of compliance, fraud-waste-and-abuse, and HIPAA training across all teams.
- Escalate and resolve clinical, compliance, or client concerns that rise above the Supervisor level, ensuring timely follow-through to resolution.

Requirements
Qualifications & Skills:
- Active Registered Nurse (RN) or higher licensure (e.g., Nurse Practitioner) in good standing with the Commonwealth of Massachusetts.
- Bachelor of Science in Nursing (BSN) required.
- Minimum of 5 years of RN experience, including home health or community-based assessment work.
- Minimum of 2 years of experience directly supervising or managing RN Supervisors, team leads, or a multi-team/multi-site nursing operation.
- Demonstrated ability to develop supervisors as leaders, including coaching-the-coach and building management capability in others.
- Strong quality-assurance and audit program experience, with the ability to implement standardized audit processes, identify trends across teams, and partner on Quality Improvement (QI) initiatives.
- Excellent leadership, coaching, communication, organizational, and reporting skills, with experience presenting performance data to internal and external stakeholders.
- Comfort documenting and working in an electronic platform via a secure virtual desktop (GuidingCare), and leading a remote, field-based, multi-team organization.
- Experience conducting or auditing assessments (for example UCA, FA, Comprehensive, MDS, or OASIS); assessment-related certification such as RAC-CT is preferred.
- Prior experience with dual-eligible populations, Medicaid, or Medicare managed care is preferred.
Working Conditions:
- Must reside within a commutable distance of the assigned Massachusetts territory.
- Field-based and hybrid; regular travel for field oversight, ride-alongs, and team support.
- Requires schedule flexibility, including occasional weekend availability or on-call escalation support to oversee a 7-day field operation.
- Exposure to weather and varied home environments during field visits.
- Valid driver's license, vehicle, and verifiable insurance are required; employment is conditional on a successful driver's license record check and verified insurance, maintained throughout employment.
- Influenza vaccination is required during flu season (October 1 through March 31) as a condition of employment, consistent with the health plan's requirement for staff serving members in the home. Employees hired during flu season must complete the required vaccination and provide proof of immunization within 30 days of hire.
