The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

$86,964.80 - $113,672.00

Position Overview

SUMMARY (Basic Purpose of the Job):

Facilitates quality performance improvement and patient safety activities of Capital Health Medical Group ambulatory practices in accordance with accreditation, regulatory, value-based care program and other requirements, including continually monitoring requirements, practices and medical staff/health system needs for purpose of modifying and monitoring activities.

Performs initial screening and analysis of quality measure performance, conducts data abstraction, and facilitates meaningful chart review and documentation compliance. Implements and monitors provider performance improvement tracking, trending and reporting for ambulatory quality programs (e.g. ACO, MIPS, HEDIS).

Analyzes trends and identifies key measures; acts upon trends and measures identified. Collaborates with practice leadership, providers, and other clinical leaders to assure compliance with regulatory and quality program requirements.

MINIMUM REQUIREMENTS

  • Education: Bachelor's degree. Master’s degree preferred. Related certification and New Jersey license Registered Nurse license preferred.

  • Experience: Three years of related healthcare experience required. Five years experience in a healthcare environment preferred. Experience with data analysis and management of clinical databases preferred.

  • Other Credentials:

  • Knowledge and Skills: Possesses knowledge of PI tool, analytics and data management. Excellent coaching, organizational, interpersonal, verbal and written communication skills. Computer and analytical skills.

  • Special Training: Intermediate to advanced computer skills.

  • Mental, Behavioral and Emotional Abilities: Conflict resolution, Ability to effectively manage multiple projects simultaneously and ability to respond quickly in a fast paced environment.

ESSENTIAL FUNCTIONS:

  • Leads, facilitates and supports performance improvement (PI) and quality initiatives and activities for assigned area to promote clinical excellence and patient safety.

  • Monitors performance on ambulatory quality programs (e.g. ACO, MIPS, HEDIS, and payer value-based care programs) and coordinates gap closure and performance improvement activities with ambulatory practices.

  • Performs analysis and trending of data through review of quality measure performance, gaps-in-care reports, and data abstraction.

  • Prepares and designs meaningful reports that include statistical analysis, interpretation, monitoring and benchmarking of clinical data.

  • Understands data definition, abstraction and benchmarking requirements to facilitate meaningful data collection, interpretation and presentation.

  • Design and maintain database system for compilation and reporting of program metrics with incorporation of benchmarks and comparative data.

  • Develops meaningful dashboards and reports for practices, departments, divisions, committees and staff.

  • Utilizes PI methodology and PI tools such as flow mapping, fish bones, and root cause analyses to facilitate and drive process improvement.

  • Develops and monitors action plans.

  • Investigates quality of care concerns from providers, payers and patients/ families.

  • Participates in other initiatives and programs and performs other duties as assigned by Director of department.

  • Attends in-service training sessions and seminars to maintain and improve knowledge of performance improvement techniques.
