Pay Range:

• Conduct RN-led Medicare Wellness Visits, completing required preventive care assessments, screenings, and Medicare visit components.
• Complete and document comprehensive patient assessments, including health history, medications, functional status, social determinants of health, safety concerns, and individualized wellness needs.
• Perform and document required screenings, including depression, cognitive, fall risk, functional, and preventive health assessments.
• Identify and support closure of preventive care gaps, including immunizations, cancer screenings, health maintenance needs, and other recommended preventive services.
• Provide patient education, health coaching, motivational interviewing, and self-management support to promote preventive care and long-term wellness.
• Develop and reinforce personalized prevention plans, including patient goals, lifestyle recommendations, preventive services, and follow-up care needs.
• Recognize abnormal findings, emerging health concerns, and care needs, escalating issues to providers and care teams according to established protocols.
• Coordinate referrals, follow-up appointments, screenings, diagnostic testing, and other care needs in collaboration with providers and interdisciplinary team members.
• Maintain accurate and timely electronic health record documentation while ensuring compliance with Medicare, CMS, coding, audit, and organizational requirements.
• Participate in quality improvement, documentation optimization, workflow standardization, professional development, and collaborative initiatives to support program performance and patient outcomes.

• Physical Requirements
  Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak English in good, understandable terms. Intact sense of sight, hearing, smell, and touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient and/or unit conditions. Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.

Education, Experience and Certifications

Graduate from an accredited School of Nursing. BSN preferred. Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works. BLS required per policy guidelines. 2 years of experience in the field of clinical specialty preferred. Professional certification in the field of specialty within 2 years if not currently certified required or eligible to meet academic professional and experiential requirement to become certified in the field of practice. Additional education, training, certifications, or experience may be required within the department by the nurse leader.
