Job Description​

We are hiring a Patient Care Navigator RN for our Neurosurgery Team at Saint Luke's Plaza and East locations!

• Full Time
• Monday - Friday. 8am - 430pm Hybrid position
• Position will round at Plaza and East locations
• MO and KS RN required
• Min 3-5 years RN experience
• Previous Neuroscience and/or Neurosurgery experience preferred.
  
  
  

The Opportunity:

• Navigates patients through the program for treatment in a timely manner. Responsible for facilitating rapid scheduling and followup tracking patients to ensure completion of their recommended treatment plan.
• Identify and problem solve any service gaps identified.
• Promote the Navigator role and treatment awareness to the community through presentations and participation in related organizations.
• Staffs multi disciplinary conference and coordinates loop closure with referring providers.
• Collects quality data on patients.
