Job Description:

Position Overview:

The New Patient Referral Specialist facilitates excellent patient care by supporting and collaborating with clinic team.

Desired/Required Qualifications:

Education:

• Required High School Diploma or equivalent
• Required to successfully complete orientation program

Specific skills & abilities:

• Preferred one year experience in an outpatient physician practice or healthcare setting

Primary Duties and Responsibilities

1. Act as a liaison between CHC and referring provider offices/hospital systems to effectively manage the intake and scheduling process of all new patients.

2. Maintain clear communication with referring entities (including self-referring patients) throughout the intake process, ensuring timely updates are provided.

3. Manage provider schedules effectively to facilitate the scheduling of new patients in a timely fashion with appropriate sub-specialty providers, while also balancing clinical urgency.

4. Maintain frequent communication with providers and leaders about schedule changes, prioritized patients, and other new patient details.

5. Finalize patient entry process by verifying demographic and insurance information, creating a new chart, and obtaining all relevant medical records.

6. Complete insurance verification and obtain global authorization (when needed) on all new patients, identifying insurance gaps or deficiencies to ensure early intervention. Identify and address potential barriers to care or access (e.g. transportation, literacy, financial, etc), referring to other teams for support when indicated.

7. Support patient portal enrollment to support value-based care measures and patient access to information.

8. Coordinates delivery and execution of pre-visit forms.

9. Maintains a dynamic wait list to track patients eligible for sooner visit times. Assists with ongoing quality improvement to provide efficient, cost-effective care.

10. Engages in continuous education and training to maintain skills, knowledge, and competency to complete functions of role.
