Position Description:
 

To assess potential, actual and ongoing needs of the patient. To provide professional discharge e planning, including, but not limited to discharge assessments, patient advocacy, resource management, utilization management, and community education, designed to meet the varying discharge needs of patients and their families as these issues relate to the current hospitalization or the subsequent transition from the acute hospital setting.

Qualifications:
Licensed Practical Nurse.
3-5 years of clinical experience preferred.
Basic computer skills.
Effective verbal and written communication skills.
Thorough assessment skills.
Demonstrated leadership ability.
Must possess excellent time-management skills and be able to prioritize work duties.
