Symbii Home Health and Hospice is seeking a Licensed Practical Nurse to join our team!

Minimal Weekends: A nurse at Symbii works an average of 1 weekend every 1-2 months and does minimal visits during their on-call weekend.

Responsibility for your patients’ care: As the LPN, you will help make decisions for your patients’ health and collaborate with the case manager, physician, family, and other members of the care team.

What will you do as an LPN at Symbii?
Provide direct patient care as defined in State’s Nurse Practice Act.
Implement plan of care initiated by the registered nurse.
Provide accurate and timely documentation consistent with the plan of care.
Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s).
Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
Participate in personal and professional growth and development.
Perform other duties as assigned by the registered nurse.

Job Requirements:
Must possess a valid LPN license in the State of Idaho
Must hold a current CPR certification
Must hold a valid driver’s license and current auto insurance
