Schedule: Monday–Friday | 8:00 AM–5:00 PM (with on call requirements)

Genesis Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the comfort of their homes.
What You’ll Do
Provide skilled nursing care according to physician orders and RN-developed plans of care
Assess and monitor patient conditions and communicate changes to the RNCM
Administer medications and treatments as prescribed
Educate patients and families on disease management, medications, safety, and preventative care
Involve patients and families in their plan of care
Complete accurate and timely clinical documentation in accordance with agency and Medicare requirements
Communicate with the care team and scheduler regarding patient needs and schedule changes
Participate in on-call coverage, case conferences, in-service training, and required meetings
Provide care within the LPN scope of practice and applicable state and federal regulations
Travel to patients’ homes within the assigned geographic area
What We’re Looking For
Current, active LPN license in the state of Ohio
Minimum of 1 year of acute care LPN experience required; 2 years preferred
Home health experience preferred
Strong communication and interpersonal skills
Compassionate, patient-focused approach
Ability to work independently, prioritize responsibilities, and make appropriate nursing judgments
Strong attention to detail and reliable work ethic
Ability to work effectively as part of a team
Valid driver’s license and auto insurance with your name listed as a driver
Education & Credentials
Graduate of an approved practical nursing program
Current, active LPN license in the state of Ohio
