Job Title: Licensed Practical Nurse - Acute General Medicine/Telemetry

Hours Per Week: Full-Time (36 hours per week)
Schedule: Day/Evening, Every Other Weekend, Every Other Holiday
SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. The Licensed Practical Nurse is a key member of the care delivery team. The Licensed Practical Nurse communicates the needs of the patient and the team; collaborates with the multidisciplinary team, takes direction from the RN and delegates appropriately to the other members of the team.
The Acute Medical/Telemetry Unit (5400/5500) is a fast-paced, high-acuity unit that cares for patients with complex medical conditions and multi-system health needs. Licensed Practical Nurses gain valuable experience managing a diverse patient population with cardiac conditions requiring telemetry, diabetes, renal disease and dialysis, pulmonary disorders, sepsis, and other acute and chronic illnesses. Nurses develop advanced clinical skills while caring for patients with multiple drips, drains, catheters, chest tubes, and feeding tubes, all while providing compassionate, patient-centered care. With strong interdisciplinary collaboration and exposure to a wide variety of diagnoses, 5400/5500 is an excellent unit for nurses looking to expand their clinical expertise and grow their careers in acute care.
RESPONSIBILITIES:
· Patient Care & Service - Promote and restore patients' health by completing the nursing process; collaborate with physicians and multidisciplinary team members; perform various treatment procedures; provide physical, educational and emotional support to patients, friends and families; supervise assigned team members.
· Medication Administration & Reporting - Administer medication and give/receive relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.
· Documentation - Ensure concise, pertinent and complete documentation using computerized medical record process.
· Preparation - Ensure clinical areas are properly supplied and exam rooms are available and stocked.
REQUIRED QUALIFICATIONS:
· Graduate from an accredited School of Practical Nursing or equivalent required.
· Current licensure in State of New York as a Licensed Practical Nurse.
PREFERRED QUALIFICATIONS:
· Strong communication, customer service, and interpersonal skills highly desired.

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
