Job Summary:

Ennoble Care is hiring for a Full-time Hospice Registered Nurse Case Manager to work within and around hospitals in Gwinnett County, GA and the surrounding areas. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers. 

Responsibilities:

Provide compassionate, patient-centered hospice nursing care primarily to patients receiving General Inpatient (GIP) level of care.

• Perform comprehensive and frequent nursing assessments for uncontrolled pain, dyspnea, agitation, nausea, seizures, bleeding, and other acute or complex symptoms.
• Develop, implement, and update individualized plans of care in collaboration with the hospice physician, interdisciplinary team, patient, family, and facility staff.
• Administer medications and treatments, including frequent medication titration, in accordance with physician orders and the hospice plan of care.
• Evaluate and document the patient’s response to interventions, changes in condition, symptom severity, and continued need for GIP services.
• Communicate promptly with the hospice physician and interdisciplinary team regarding changes in condition, ineffective interventions, and recommendations for additional symptom-management measures.
• Educate patients and families about the dying process, medication use, symptom management, goals of care, and what to expect during the GIP stay.
• Coordinate care with hospital, skilled nursing facility, or hospice inpatient-unit staff to ensure continuity, safety, and compliance with the hospice plan of care.
• Participate in daily evaluation and discharge planning, facilitating a safe transition to Routine Home Care or another appropriate level once symptoms are stabilized.
• Maintain timely, accurate, and compliant clinical documentation demonstrating the interventions provided, patient outcomes, care coordination, and medical necessity for continued GIP care.

Qualifications:  

• Possession of a current license to practice as a registered nurse in the State of GA
• At least 1-2 years of work experience as a professional nurse preferably at least one of the years in hospice care
• Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
• Skilled in nursing practice, able to cope with loved ones/caregivers’ emotional stress and tolerant of individual lifestyles
• Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports
• Must be a team player who supports an interdisciplinary team concept of caring for patient and loved ones/caregivers
• Must have a valid driver’s license, a good driving record, the use of a reliable vehicle, and the ability to drive year-round to locations
• Must be able to take direction, work independently and effectively as part of a multidisciplinary team in a positive, professional manner which supports the mission of the organization

Starting Pay: $85,000 annually
