Job Description:

Certified Hemodialysis Technician

The Certified Hemodialysis Technician (CHDT) supports the delivery of safe, effective, and patient-centered hemodialysis services at Bay Area Hospital. Under the direct supervision of a Registered Nurse, physician, or nurse practitioner, the CHDT assists with the preparation, initiation, monitoring, and discontinuation of hemodialysis treatments within the technician’s authorized scope of practice.

The CHDT works collaboratively with the Dialysis RN and interdisciplinary care team to monitor and communicate patient information, maintain dialysis equipment and treatment areas, perform required equipment and water-quality testing, and support the safe and effective operation of Bay Area Hospital’s dialysis program.

PRINCIPLE DUTIES AND RESPONSIBILITIES

· Provides dialysis-related patient care under the direct supervision of a Registered Nurse, physician, or nurse practitioner and within the scope permitted by Oregon law, Hospital policy, demonstrated competency, and assigned responsibilities.

· Prepares patients and dialysis equipment for treatment in accordance with established policies, procedures, infection prevention standards, and manufacturer requirements.

· Obtains and records pre-treatment and post-treatment patient information, including weight, vital signs, patient complaints, and other assigned clinical data, and promptly communicates findings and changes in patient condition to the Registered Nurse.

· Performs venipuncture for dialysis access within authorized scope of practice and demonstrated competency.

· Connects and disconnects dialysis access and assists with initiation and discontinuation of dialysis treatment within authorized scope of practice and Hospital policy.

· Initiates or discontinues dialysis treatment via central lines when permitted by Oregon law, Hospital policy, and demonstrated competency.

· Obtains and administers intradermal lidocaine, heparin bolus, normal saline bolus, and oxygen when permitted by Oregon law and in accordance with applicable orders, protocols, Hospital policy, and demonstrated competency.

· Monitors patients throughout dialysis treatment and promptly reports changes in vital signs, symptoms, vascular access, treatment parameters, or patient condition to the supervising Registered Nurse.

· Monitors dialysis equipment during treatment and immediately reports alarms, equipment concerns, treatment interruptions, or results outside established parameters to the Registered Nurse.

· Assists the Dialysis RN in responding to patient complications and emergency situations within established training, competency, and scope of practice.

· Performs appropriate dialysis machine setup, testing, operation, cleaning, disinfection, and preparation for subsequent treatment in accordance with manufacturer instructions and Hospital policy.

· Performs assigned dialysis water treatment system monitoring and testing, including applicable chlorine/chloramine, hardness, conductivity, and other required safety testing in accordance with established procedures and demonstrated competency.

· Documents water-quality and equipment testing results accurately and immediately reports results outside established parameters.

· Maintains dialysis equipment, supplies, and treatment areas in a clean, organized, and treatment-ready condition.

· Follows infection prevention practices specific to dialysis care, including hand hygiene, appropriate use of personal protective equipment, bloodborne pathogen precautions, vascular access precautions, environmental cleaning, and equipment disinfection.

· Assists with inventory management, stocking, equipment checks, and identification of supply or equipment needs.

· Documents patient care activities, treatment information, equipment testing, and other required information accurately and timely in the appropriate medical record or departmental system.

· Reinforces patient education previously established by the Registered Nurse and directs clinical questions or education needs to the appropriate licensed clinician.

· Communicates respectfully and effectively with patients, families, nurses, physicians, and other members of the interdisciplinary care team.

· Participates in department orientation, continuing education, competency validation, safety activities, quality improvement initiatives, and regulatory readiness activities.

· Maintains required certification and completes ongoing education and competency requirements established by Bay Area Hospital and applicable regulatory agencies.

· Identifies and promptly reports patient safety, equipment, infection prevention, water-quality, or operational concerns through appropriate Hospital channels.

· Participates in on-call, weekend, holiday, and other scheduling requirements based on departmental and patient care needs.

· Performs other duties as assigned within authorized scope of practice.

SCOPE OF PRACTICE

The Certified Hemodialysis Technician performs assigned duties only within the scope permitted by Oregon law, Bay Area Hospital policy, demonstrated competency, and under required clinical supervision.

The CHDT does not independently assess patients, develop or modify the nursing plan of care, determine the frequency or duration of dialysis treatment, alter treatment prescribed by a provider, accept verbal or telephone provider orders, administer blood or blood products, or administer medications outside those specifically authorized within the CHDT scope of practice.

The CHDT does not perform hemodialysis on hospitalized intensive care unit patients.

Bay Area Hospital may establish a scope of practice that is more restrictive than the maximum scope permitted under Oregon law.

SKILLS AND ABILITIES

· Demonstrates knowledge of basic hemodialysis principles, dialysis equipment, vascular access, infection prevention, and patient safety practices.

· Demonstrates competency in assigned dialysis procedures, equipment operation, water-quality testing, and treatment-related documentation.

· Recognizes and promptly communicates changes in patient condition or dialysis treatment parameters to the supervising Registered Nurse.

· Understands and consistently works within the established scope of practice and seeks RN guidance when patient needs or circumstances exceed that scope.

· Demonstrates effective organization and the ability to prioritize responsibilities in a changing acute-care environment.

· Communicates clearly, respectfully, and collaboratively with patients, families, physicians, nursing staff, and other members of the interdisciplinary team.

· Demonstrates attention to detail and follows established clinical, infection prevention, equipment, and safety procedures.

· Maintains regular, consistent, and punctual attendance at the assigned job location.

EDUCATION/CERTIFICATIONS/LICENSES/DEGREES

· High school diploma or equivalent, required

· Current Oregon hemodialysis technician certification or provisional certification as required by the Oregon Health Authority, required

· Current American Heart Association Healthcare Provider Basic Life Support (BLS) certification, required

EXPERIENCE

· Previous experience in a healthcare or direct patient care setting, preferred

· Previous hemodialysis technician or dialysis patient care experience, preferred

· Acute-care or hospital experience, preferred

· Experience with hemodialysis equipment, vascular access, infection prevention, and dialysis water treatment systems, preferred

· Ability to maintain sustained attention and recognize and promptly communicate changes in patient condition, equipment function, treatment parameters, or water-quality results.

· Regular exposure to blood, body fluids, blood products, medications, infectious or contagious diseases, cleaning and disinfecting agents, and other materials commonly encountered in an acute healthcare and dialysis environment; appropriate PPE is required.

· Ability to work in an environment involving medical equipment, alarms, moderate noise, frequent interruptions, computer use, and regular interaction with patients, families, physicians, and staff.

· Ability to participate in departmental scheduling requirements, including on-call coverage, weekends, and holidays, based on patient care and operational needs.
