Reporting to the Nurse Manager/Director of the Unit, provides professional nursing care within assigned unit. Coordinates care planning with other disciplines.
Major Responsibilities: Practice Standards
- Adheres to hospital policies, ANA Standards of Practice, and the Nursing Code of Ethics.
- Ensures safe, timely, and accurate medication administration, IV administration, and transcription.
- Effectively utilizes information technology in clinical practice.
- Maintains documentation that is accurate, timely, concise, legible, and compliant with departmental policies.
- Integrates organizational policies and national practice guidelines into patient care planning to achieve desired outcomes.
- Serves as an expert resource for clinical practice content development, including policy and procedure review or development.
Mentoring and Teaching
- Precepts new employees by establishing goals and assisting them in achieving those goals.
- Facilitates and encourages an environment that promotes critical thinking.
- Delivers 1 to 2 formal presentations annually to peers, such as: Case studies New equipment in-services Educational programs
- Supports the learning and professional development needs of colleagues.
Delegation
- Understands team members' roles and appropriately delegates tasks and responsibilities based on licensure, education, and experience.
- Assigns personnel effectively and recognizes when follow-up is necessary.
- Monitors and evaluates appropriate staff utilization during assigned shifts.
Assessment
- Performs comprehensive physical assessments and obtains patient histories upon admission, appropriate to the clinical setting and patient condition.
- Conducts systematic, ongoing assessments based on patient condition and unit standards.
- Utilizes evidence-based assessment techniques and tools to collect relevant patient data.
Prioritization and Organization
- Prioritizes patient care problems and needs effectively.
- Makes sound clinical decisions independently and collaboratively while managing assigned patient care responsibilities.
- Balances efficiency and cost management within assigned workloads.
- Recognizes personal limitations and seeks assistance or collaboration when needed.
- Adjusts plans and reprioritizes work in response to unexpected situations.
- Manages multiple demands while prioritizing the patient care needs of an entire unit or department.
- Adapts established protocols and available resources to support effective clinical decision-making in less predictable situations.
Planning and Implementation
- Communicates effectively during all patient hand-offs.
- Anticipates and identifies patient care needs and initiates appropriate plans of care based on actual or potential health risks.
- Establishes mutually agreed-upon patient care outcomes.
- Identifies patient priorities and discharge planning needs using: Patient assessments Best-practice standards Multidisciplinary team input Patient and family input
Evaluation
- Evaluates and modifies plans of care based on patient outcomes and feedback.
- Assesses the effectiveness of interventions and implements adjustments as needed.
Patient and Family Education
- Identifies patient education needs and collaborates with healthcare professionals to provide appropriate learning opportunities.
- Documents patient education, interventions, and patient responses clearly and concisely.
- Independently develops formal patient education materials.
- Develops and participates in community education programs and health screenings.
Professional Growth and Development
- Pursues formal education to maintain and advance clinical and professional knowledge and skills.
- Seeks assignments that provide growth, challenge, and expanded responsibility.
- Anticipates the need for and acquires new skills and training.
- Demonstrates the ability to work across multiple specialty areas, units, or care environments.
- Maintains active membership in professional organizations and shares relevant information with the department.
- Maintains specialty certifications as required.
Requirements:
- RN license - N.H. state or any other compact state.
- BCLS certified or within 30 days of hire; BSN strongly preferred
- ACLS certified or within 1 year of hire
Additional Skills & Qualifications: BSN preferred
Pay Range:
$35.00 - $63.15The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
