About the Role:
Amazing Care Home Health Services is seeking an experienced Adult Therapy Supervisor to provide clinical leadership and operational oversight for our adult home health therapy services in Colorado.
This role serves as a key connection point between our patients, field clinicians, care management, and operational teams. The Adult Therapy Supervisor is responsible for supporting a team of Physical Therapists, Occupational Therapists, and Speech-Language Pathologists while helping ensure patients receive timely, coordinated, high-quality care throughout the entire episode of service.
This is an ideal opportunity for an experienced home health therapist who enjoys developing clinicians, solving operational challenges, improving care delivery, and making an impact beyond an individual patient caseload.
What You'll Do:
Clinical Leadership & Team Support
- Provide day-to-day clinical leadership, guidance, and support to a multidisciplinary team of PTs, OTs, and SLPs.
- Serve as a clinical resource for therapists navigating patient needs, changes in condition, care planning, documentation, and other clinical questions.
- Support clinician development through coaching, education, feedback, and ongoing communication.
- Promote clinical excellence, accountability, collaboration, and a strong patient-first culture across the therapy team.
- Partner with clinicians to identify and resolve barriers impacting patient care or successful service delivery.
- Support onboarding, orientation, and ongoing development of new therapy team members.
- Assist with patient visits and clinical coverage when needed to support continuity of care.
Patient Care & Case Management
- Oversee and coordinate therapy services throughout the patient lifecycle, from admission through discharge.
- Help ensure appropriate and timely delivery of therapy services based on patient needs and established plans of care.
- Collaborate with clinicians and internal teams to address changes in patient status, scheduling challenges, missed visits, or other barriers to care.
- Support effective case management and communication across disciplines.
- Collaborate with physicians, physician offices, care managers, field clinicians, patients, families, and internal partners to facilitate coordinated care.
- Support appropriate utilization of therapy services while maintaining a strong focus on patient outcomes and quality.
Quality, Documentation & Compliance
- Provide oversight and support related to clinical documentation, including OASIS requirements as applicable.
- Help ensure therapy documentation is timely, accurate, complete, and aligned with regulatory, payer, and organizational requirements.
- Identify documentation or clinical practice trends and provide coaching or education when opportunities for improvement are identified.
- Reinforce adherence to established clinical policies, procedures, standards of practice, and applicable home health regulations.
- Partner with clinical leadership and compliance resources to address quality or documentation concerns when needed.
- Promote continued readiness for audits, surveys, and other regulatory requirements.
Operational Leadership
- Partner closely with clinical, scheduling, care management, recruiting, and operational teams to ensure effective patient coverage and clinician utilization.
- Help identify gaps in coverage, workflow, communication, or care delivery and proactively drive solutions.
- Maintain visibility into assigned patient and clinician needs and escalate barriers appropriately.
- Balance patient needs, clinician capacity, operational priorities, and regulatory requirements when making decisions.
- Participate in team meetings and cross-functional initiatives focused on improving patient care, clinician experience, quality, and operational performance.
- Contribute to process improvements as the organization and adult home health service line continue to grow.
Requirements
- Current, unrestricted Colorado license as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
- Professional experience providing therapy services to adult and/or geriatric populations.
- Home health experience with a strong understanding of the unique clinical and operational needs of adult therapy-based care delivered in the home.
- Experience with OASIS documentation and home health clinical documentation requirements.
- Strong clinical judgment and ability to independently evaluate and navigate patient-care concerns.
- Excellent communication, organization, and time-management skills.
- Ability to effectively prioritize competing clinical and operational needs in a fast-paced environment.
- Strong interpersonal skills with the ability to build productive relationships with clinicians, patients, families, physicians, and cross-functional partners.
- Comfortable working independently while remaining highly collaborative and team-oriented.
Preferred Qualifications
- Previous supervisory, lead, preceptor, or clinical leadership experience.
- Experience coaching, mentoring, or educating therapy clinicians.
- Experience overseeing a multidisciplinary therapy team.
- Strong working knowledge of Medicare-certified home health operations, documentation, and regulatory expectations.
- Experience supporting quality improvement, clinical compliance, or operational initiatives within home health.

Role Type: This a Full-Time Hybrid role based out of your preferred Colorado office location: Aurora, Loveland, Westminster, or Colorado Springs.
Compensation: $90,000 - $105,000 - *commensurate with experience
