Job Description:

The Student Wellness Case Management Coordinator supports students experiencing complex mental health and wellbeing-related concerns that impact their ability to access, engage with, and benefit from available campus and community supports.

The incumbent provides non-clinical case management services through support planning, service navigation, advocacy, resource coordination, outreach, and ongoing engagement. Working collaboratively within and outside of Student Support Services including collaboration with Counselling Services, Health Services, The Centre for Accessible Learning, Housing Services, Student Success Specialists, faculty and community partners, the Student Wellness Case Management Coordinator serves as a central point of coordination for students requiring support from multiple services and systems.

The role assists students in identifying goals, reducing barriers to service access, navigating campus and community resources, developing self-advocacy skills, and maintaining engagement with supports that promote wellbeing and engagement.

The Student Wellness Case Management Coordinator does not provide counselling, psychotherapy, diagnosis, clinical assessment, treatment planning, or therapeutic intervention. The role complements the work of teams and departments through coordinated support and service navigation activities.

Required Qualifications:

Education & Training

• Minimum three (3) year diploma or degree in Social Work, Psychology, Human Services, Community and Justice Services, Sociology, or related discipline. A Social Service Worker diploma combined with significant directly related experience may be considered equivalent.
• Registration with OCSWSSW is considered a strong asset.

Experience

Minimum of three (3) years’ experience in:

• Mental health case management and wellbeing support.
• Community resource mapping, referral coordination, and service navigation.
• Supporting post-secondary students or young adults (preferred).

This position requires regular on-site work at Algonquin College’s Woodroffe (Ottawa) Campus.
