Liquid Personnel is recruiting an Adults Social Worker to join its client’s Rapid Discharge to Assess Team based in Trafford.

What will your responsibilities be?

• Timely Hospital Discharge Planning: Work closely with health professionals to assess patients' needs and coordinate safe, efficient discharge plans that avoid unnecessary hospital stays.
• Holistic Needs Assessment: Conduct strength-based assessments in community settings post-discharge to determine ongoing care and support requirements, ensuring person-centred outcomes.
• Multi-Agency Collaboration: Liaise with families, care providers, and partner agencies to implement tailored support packages and facilitate smooth transitions from hospital to home or interim care.

Qualifications & Experience:

To be successful in this role you must have:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
