Liquid Personnel is recruiting Social Worker for its client's Older Adults Team at Ceredigion.
This is an excellent opportunity for a dedicated practitioner to support older adults with a range of care and support needs, promoting independence, wellbeing, and positive outcomes through strengths-based and person-centred social work practice.
The successful candidate will work closely with older adults, carers, families, health professionals, and partner agencies to review and reassess care and support arrangements, ensuring services remain appropriate, effective, and outcome focused. You will play a key role in supporting individuals to maintain their independence, remain connected to their communities, and receive the right support at the right time.
This role requires a confident Social Worker who can manage a varied caseload, undertake complex assessments and reviews, and apply professional judgement within the framework of the Social Services and Well-being (Wales) Act 2014 and associated legislation.
What will your responsibilities be?
- Managing a caseload of older adults with a range of care and support needs.
- Undertaking reviews and reassessments of Care and Support Plans to ensure services remain appropriate, proportionate, and outcome-focused.
- Completing strengths-based assessments in accordance with the Social Services and Well-being (Wales) Act 2014.
- Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
- Working collaboratively with individuals, families, carers, health professionals, and partner agencies to coordinate effective support.
- Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making processes where required.
- Assessing and managing risk appropriately, balancing individual choice with safety and positive risk-taking.
- Undertaking safeguarding enquiries and responding to concerns relating to abuse, neglect, exploitation, and self-neglect.
- Supporting hospital discharges, community-based interventions, and preventative approaches where required.
- Promoting access to community resources, support networks, and services that enhance independence and reduce social isolation.
- Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences.
- Preparing high-quality assessments, reviews, reports, and professional documentation.
- Maintaining accurate, timely, and legally compliant case records in line with organisational requirements.
Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
