What will your responsibilities be?

In this role, you will:

• Provide statutory social work support, delivering professional social work interventions and fulfilling legal duties under adult social care legislation.
• Promote independence and recovery by completing strengths-based assessments and supporting individuals to achieve the best possible outcomes.
• Coordinate safe hospital discharges, working closely with health partners to ensure timely, appropriate discharge planning and effective community support.
