Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Hospital team, based in Wolverhampton. This is an excellent opportunity for a dedicated practitioner to support adults with complex care and support needs through safe, timely, and person-centred discharge planning from hospital settings.
The successful candidate will work within a fast-paced multidisciplinary environment alongside health professionals, therapists, discharge coordinators, carers, families, and community-based services. You will play a key role in ensuring individuals receive the right support at the right time, promoting independence, wellbeing, and positive outcomes whilst reducing unnecessary delays in discharge.
This role requires a confident and experienced Social Worker with strong assessment skills, safeguarding knowledge, and a sound understanding of Adult Social Care legislation. You will be responsible for managing complex cases, coordinating services, and supporting adults through periods of transition from hospital back into their communities.
What will your responsibilities be?
- Managing a caseload of adults requiring social care intervention within acute and community hospital settings.
- Undertaking Care Act 2014 assessments, reviews, and reassessments to identify eligible needs and support discharge planning.
- Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and positive outcomes.
- Supporting safe, effective, and timely hospital discharges through collaborative planning and intervention.
- Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Assessing and managing risk, balancing individual choice, control, and independence with safeguarding responsibilities.
- Undertaking safeguarding enquiries and responding to safeguarding concerns in accordance with Adult Safeguarding legislation and local procedures.
- Arranging and coordinating care packages, residential placements, equipment, reablement services, and community-based support where required.
- Working collaboratively with health professionals, community teams, care providers, carers, families, housing services, and partner agencies.
- Supporting adults and carers through periods of change and transition, offering advice, guidance, and advocacy where appropriate.
- Participating in multidisciplinary meetings, discharge planning meetings, safeguarding discussions, and case reviews.
- Maintaining accurate, timely, and legally compliant assessments, reports, case records, and support plans.
- Promoting strengths-based practice and supporting individuals to maximise their independence following hospital admission.
Qualifications and Experience:
To be successful in this role you must have;
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Hospital Experience: Previous experience within a Hospital Team, Hospital Discharge Team, Integrated Discharge Service, or similar Adult Social Care setting is highly desirable.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake Care Act assessments, Mental Capacity Assessments, support planning, and risk management interventions.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and managing complex safeguarding concerns.
- Multi-Agency Working: Strong experience working collaboratively with NHS professionals, therapists, care providers, housing services, community organisations, families, and carers.
- Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
- Organisational Skills: Ability to manage a complex caseload, prioritise competing demands, and work effectively within a fast-paced hospital environment.
