Liquid Personnel is recruiting Social Worker for its client's Community Team at Worcestershire.

This is an excellent opportunity for a dedicated practitioner to work with adults across a varied caseload, delivering strengths-based, person-centred interventions that promote independence, wellbeing, and positive outcomes.

The successful candidate will undertake Care Act assessments, reviews, safeguarding enquiries, and Mental Capacity Assessments, supporting adults with a wide range of needs within the community. Working collaboratively with individuals, families, carers, health professionals, and partner agencies, you will ensure that people receive appropriate support and services that maximise choice, control, and independence whilst remaining within their local communities.

What will your responsibilities be?

Core duties include:

• Managing a caseload of adults with varying levels of need, undertaking assessments and interventions in accordance with statutory duties and best practice.
• Completing strengths-based, person-centred Care Act assessments to identify eligible needs, desired outcomes, and appropriate support arrangements.
• Undertaking scheduled and unplanned reviews of care and support plans to ensure services remain effective, proportionate, and outcome focused.
• Conducting Mental Capacity Assessments and applying the principles of the Mental Capacity Act 2005 to support decision-making and protect individual rights.
• Completing safeguarding enquiries and interventions in accordance with Section 42 of the Care Act 2014, ensuring adults are protected from abuse, neglect, self-neglect, and exploitation.
• Assessing and managing risk, implementing appropriate safeguarding measures and support arrangements where required.
• Developing and reviewing person-centred care and support plans that promote independence, wellbeing, choice, and community inclusion.
• Working closely with individuals, carers, families, advocates, and partner agencies to identify strengths, build resilience, and achieve positive outcomes.
• Supporting individuals through periods of crisis, utilising professional judgement, available resources, and relevant legislation to achieve timely resolutions.
• Coordinating services with health professionals, care providers, voluntary organisations, and community resources to meet assessed needs.
• Undertaking carers' assessments where appropriate and ensuring carers are signposted to relevant support services.
• Preparing clear, accurate, and comprehensive assessments, reports, case notes, and support plans in line with professional and organisational standards.
• Maintaining accurate and up-to-date case records and ensuring compliance with legislative requirements and local procedures.
• Participating in multi-disciplinary and multi-agency meetings to ensure coordinated and effective support for individuals.

Qualifications & Experience:

To be successful in this role you must have, 

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
• Registration: Must be registered with Social Work England.
• Experience: Significant post-qualified experience within Adult Social Care, preferably within a Community Team setting.
• Legislation Knowledge: Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
• Assessment Skills: Demonstrable experience completing strengths-based Care Act assessments, reviews, risk assessments, and support planning.
• Mental Capacity Experience: Experience undertaking Mental Capacity Assessments and applying best-interest decision-making processes where required.
• Safeguarding Experience: Proven ability to identify, investigate, and manage safeguarding concerns involving vulnerable adults.
• Multi-Agency Working: Ability to work effectively with health professionals, providers, carers, families, and partner organisations.
• Communication Skills: Excellent communication, report-writing, assessment, analytical, and decision-making skills.
• Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
