The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

The embedded medical social worker is an integral member of the Medicine in Psychiatry Service – Primary Care (MIPS-PC) outpatient clinic- located at Brighton Health Campus. Social work services provided include biopsychosocial assessments and treatment, advocacy, care coordination and other services to adult patients and their families, with the goal of improving the patient’s overall health and well-being.

In this role, the medical social worker partners with the primary care team and provides screening of patients with medical comorbidities and psychiatric conditions in the development and implementation of the care plan in this clinic.

The position requires consultation, education and interdisciplinary collaboration with clinicians and health care providers and staff across MIPS care teams and/or interorganizational collaboration with community agencies involved in supporting patients’ care needs.

MIPS-PC provides full-spectrum primary care services for adults (ages 18 and older) with severe mental illness and/or chemical dependency issues who are linked with psychiatric care within the University of Rochester Medicine or other regional psychiatric care teams.

ESSENTIAL FUNCTIONS

  • Performs screening and biopsychosocial assessment of patients and families referred by clinic providers or identified through high-risk screening. These patients include but are not limited to; adult, elderly and/or disabled persons in need of home services or protection; persons experiencing emotional crises; and patients with new, chronic or terminal diagnoses. Maintains current knowledge of community resources relevant to the clinic patient population.  Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence.
  • Provides social work service to patients with problems identified through biopsychosocial assessment. Treatment may include biopsychosocial support and education, health promotion, supportive counseling, trauma-informed care; referral; respite or long-term facility-based care; resource navigation and assistance with accessing resources; crisis intervention and emergency support. Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence.  Services are largely provided within the ambulatory clinic
  • Provides internal and external systems advocacy across continuums of care for identified patients. Collaborates with clinic care providers in the development of and implementation of interdisciplinary care plans. Collaborates with appropriate collateral contacts and provides linkages to community resources, acts as liaison to community agencies and within UR Medicine/SMH to ensure continuity of care.
  • Collaborates closely with inpatient social work teams (particularly SW teams in 19200 and G9200, but other inpatient teams as well) to facilitate transitions of care from hospital to home and follow up in the MIPS-PC office.
  • Documents social work services and interventions in patient’s electronic medical record in compliance with Division, Department and Hospital Policy.
  • Provides crisis intervention and emergency support during working hours for assigned and/or referred patients of the practice, as indicated.
  • Attends Social Work Division and Program staff meetings. Participates in activities of the Social Work and Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented about professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. Subscribes to the departmental goal of creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
  • Maintains a thorough knowledge of, and follows all Discipline, Division, and Department policies and procedures. Maintains professional standards and continuing education by reading, attending seminars, workshops, and conferences, and staying up to date for the NYS requirements for maintenance of licensure as appropriate. Engages in annual and periodic performance review process with Medical/Program Director, discipline supervisor, and clinical chief, where relevant, for the purposes of recognition of strengths and contributions, feedback, and continuous quality improvement relative to performance in the role as well as the service needs.
  • May supervise social work students (MSW or BSW), as approved by social work supervisor. Other duties as assigned


MINIMUM EDUCATION & EXPERIENCE

  • Master's degree from an accredited school of social work required
  • Or equivalent combination of education and experience
  • Prior experience in a medical setting, psychiatric, crisis or outreach work; or an equivalent combination of education and experience and working with adult populations. Prior medical social work in a primary care setting preferred.


KNOWLEDGE, SKILLS AND ABILITIES

  • Previous experience with medical comorbidities and psychiatric conditions preferred
  • Commitment to and routine demonstration of biopsychosocial approach to care for individuals and families preferred.
  • Dedication to delivering a team-based integrated model of care preferred.
  • Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome preferred.


LICENSES AND CERTIFICATIONS

  • LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or
  • If duties will be performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required
