Liquid Personnel is recruiting a Social Worker for its client's Adult D2A Hospital Discharge Team at Blackburn with Darwen 

What will your responsibilities be?

• Complete strengths‑based Care Act assessments to identify needs and plan safe, timely discharges through D2A pathways.
• Work closely with hospital teams, community services, and families to coordinate person‑centred discharge plans that promote independence and recovery.
• Develop, implement, and review support plans, managing risk appropriately and ensuring adults are safeguarded throughout the discharge process.

Qualifications & Experience:

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
