What You’ll Do

As a Behavioral Health Registered Nurse, you will:

• Assess patients’ physical, behavioral, psychosocial, and environmental needs.

• Develop, coordinate, implement, and evaluate individualized plans of care.

• Manage an assigned caseload and complete initial and ongoing nursing assessments.

• Monitor patients for changes in condition, safety concerns, and barriers to treatment.

• Communicate important updates to physicians, clinical leadership, caregivers, and other members of the care team.

• Educate patients and caregivers about medications, symptom management, safety, and treatment goals.

• Update plans of care based on patients’ changing needs and progress.

• Coordinate services with an interdisciplinary team to support positive patient outcomes.

• Supervise and document care provided by LPNs and behavioral health aides when applicable.

• Maintain accurate and timely clinical documentation.

What You’ll Need

• Current, unrestricted RN license in the state of practice

• Associate or bachelor’s degree in nursing

• At least one year of clinical nursing experience

• Behavioral health, home health, or hospice experience preferred

• Homecare HomeBase experience preferred

• Dependable vehicle, valid driver’s license, and current automobile insurance

• Willingness to travel throughout the assigned territory

• Ability to travel up to 50%

• Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
