PHYSICAL THERAPIST (PT)

SCHEDULE: Full-Time, 5 days per week, Monday-Friday, 8:30 AM-5:00 PM, + Weekend On Call Requirements.

TERRITORY: Mt Vernon, Kingstown, Arlington, and Alexandria City.

AGENCY: Alternate Solutions Home Health.

HOW YOU'LL MAKE A DIFFERENCE:

As a Home Health Physical Therapist (PT), you will provide skilled physical therapy services under physician orders to help patients regain mobility, restore function, reduce pain, and maximize independence in the comfort of their homes.

HOW YOU'LL WORK:

You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

MAJOR AREAS OF RESPONSIBILITY:

Patient Care: Perform comprehensive patient evaluations, including OASIS assessments, and develop individualized plans of care. Provide skilled PT treatments that improve mobility, strength, balance, function, and overall quality of life. Complete initial evaluations and admissions within established agency timeframes. Modify treatment plans based on patient response and changing clinical needs. Help reduce hospital readmissions through early intervention, patient education, and proactive care planning. Coordinate outpatient services when appropriate to support continuity of care.

Patient & Family Education: Educate patients and caregivers on disease management, home exercise programs, fall prevention, mobility techniques, and home safety. Instruct patients and families on the safe use of walkers, canes, crutches, wheelchairs, braces, prosthetics, orthotics, and other adaptive equipment. Encourage patient participation in developing and achieving therapy goals. Collaborate with the DME team and other resources to ensure patients understand equipment needs and financial responsibilities.

Care Coordination: Collaborate with physicians, nurses, PTAs, and other interdisciplinary team members to ensure coordinated, high-quality care. Supervise PTAs and complete required supervisory visits. Communicate patient progress, changes in condition, and scheduling updates with the care team. Participate in case conferences, interdisciplinary meetings, and on-call responsibilities as assigned.

Documentation & Compliance: Complete accurate and timely documentation in accordance with agency policies, Medicare guidelines, and state and federal regulations. Document evaluations, treatments, patient progress, assessments, and education. Participate in quality assurance activities and chart audits as requested. Maintain an active PT license and remain compliant with all regulatory requirements.

Professional Responsibilities: Attend mandatory meetings, in-service education, and continuing education programs. Assist with onboarding and mentoring new clinicians as needed. Maintain a professional appearance and represent the organization positively. Follow all agency policies, procedures, and standards of practice.

• Attention to detail is critical, as is being observant and following directions
• Home care experience preferred.

REQUIREMENTS:

• Physical Therapist Assistant (PT) with current license in the state of employment.
• Minimum one-year experience as a Physical Therapist (PT) in an acute care setting.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
