H2 Health is seeking a Physical Therapist Assistant to join our outpatient team in Murray, Kentucky. At H2 Health, you'll work with a diverse outpatient population and play an important role in helping patients recover from injury, surgery, and a variety of musculoskeletal and mobility-related conditions. You'll work closely with Physical Therapists and support staff to provide individualized, evidence-based care while building meaningful relationships with patients throughout their rehabilitation journey. Your Role:
- Provide skilled physical therapy treatment under the direction of a licensed Physical Therapist.
- Implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Treat orthopedic, post-operative, musculoskeletal, and mobility-related conditions.
- Educate patients and caregivers on home exercise programs and injury prevention strategies.
- Accurately document patient progress and communicate treatment updates with the clinical team.
- Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
- Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
- Collaborate with Physical Therapists and support staff to support strong patient outcomes.
- Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
- Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
Requirements
- Associate degree from a CAPTE-accredited Physical Therapist Assistant program.
- Current licensure or eligibility as a Physical Therapist Assistant.
- Eligible to work in the United States.
- Experience in outpatient orthopedic, musculoskeletal, post-operative, or sports rehabilitation settings is a plus.
- Experience with EMR documentation, scheduling workflows, and billing-related documentation is a plus.
