Job Description:

Patient Safety Assistant

The Patient Safety Assistant (PSA) provides continuous observation of patients who require close monitoring due to medical, behavioral, or safety concerns. Working under the direction of the Registered Nurse (RN), the PSA promotes a safe environment by maintaining constant observation, identifying and communicating changes in patient behavior or condition, and supporting established safety interventions. The PSA helps ensure the safety of patients, visitors, and staff while adhering to Bay Area Hospital’s mission, vision, values, policies, procedures, and regulatory requirements.

POSITION SUMMARY

The Patient Safety Assistant (PSA) provides continuous observation of patients who require close monitoring due to medical, behavioral, or safety concerns. Working under the direction of the Registered Nurse (RN), the PSA promotes a safe environment by maintaining constant observation, identifying and communicating changes in patient behavior or condition, and supporting established safety interventions. The PSA helps ensure the safety of patients, visitors, and staff while adhering to Bay Area Hospital’s mission, vision, values, policies, procedures, and regulatory requirements.

PRINCIPLE DUTIES AND RESPONSIBILITIES

  • Provides continuous, uninterrupted observation of assigned patients requiring close monitoring
  • Maintains a safe environment by conducting room safety checks, identifying potential hazards, and promptly notifying the RN of safety concerns or changes in the patient’s behavior, condition, or activity
  • Remains alert and attentive at all times, intervening only within the scope of the role to support patient safety and immediately notifying the RN of any concerns or unsafe behaviors
  • Implements established patient safety interventions and follows individualized safety plans as directed by the RN
  • Accurately documents required patient observations, interventions, patient responses and other assigned activities in accordance with hospital policy and established procedures
  • Communicates effectively and professionally with patients, families, guests and other members of the care team to promote a safe, respectful, and therapeutic environment
  • Demonstrates professionalism by protecting patient privacy and confidentiality and respecting the rights, dignity, and individual needs of patients and families
  • Provides a calm, supportive, and therapeutic presence while maintaining appropriate professional boundaries
  • Participates in maintaining a safe, clean, and organized patient care environment
  • Performs other duties within the scope of the PSA role as assigned

SKILLS AND ABILITIES

  • Maintains regular, reliable, and punctual attendance
  • Strong observation skills and attention to detail
  • Effective communication and interpersonal skills
  • Ability to remain calm and composed in stressful or rapidly changing situations
  • Ability to stand or sit for extended periods
  • Follows verbal and written instructions, hospital policies, procedures and established safety protocols
  • Must be able to wear appropriate personal protective equipment (PPE), if indicated

EDUCATION/CERTIFICATIONS/LICENSES/DEGREES

  • High School Diploma or Equivalent required.
  • American Heart Association Healthcare Provider Basic Life Support (BLS) required
  • Organization approved workplace violence training required or obtained by the last day of the month after 90 day grace period.

EXPERIENCE

  • Healthcare experience preferred
